摘要
目的探究CT主肺动脉直径(dPA)、主肺动脉与升主动脉直径比值(rPA)、肺动脉高压的相关性。方法 120例疑似肺动脉高压患者作为研究对象,所有患者给予肺动脉计算机断层造影成像(CTA),对dPA及升主动脉直径进行测量得出rPA,对患者肺动脉平均压(mPAP)予以右心导管检查,对dPA、rPA与mPAP的相关性进行分析。结果经过右心导管检查,120例患者中102例确诊为肺动脉高压,18例肺动脉压正常,肺动脉高压患者dPA、rPA显著高于肺动脉压力正常者(P<0.05);经Pearson分析,dPA、rPA与mPAP均呈现出轻度相关性(P<0.05)。将dPA 30 mm作为界值,其诊断肺动脉高压的敏感度、特异度分别为90.7%、66.8%;将rPA 1.0作为界值,其诊断肺动脉高压敏感度、特异度分别为94.4%、55.7%。结论肺动脉高压患者dPA、rPA与肺动脉平均压具有相关性,可以作为肺动脉高压的重要预测指标,具有较高临床诊断价值。
Objective To study the relationship between the diameter of main pulmonary artery,the ratio of main pulmonary artery to ascending aorta diameter and the pulmonary hypertension.Methods 120 cases of patients with suspected pulmonary hypertension were collected and detected the CT angiography(CTA).The rPA was calculated on account of the dPA and ascending aorta diameter.Average pulmonary artery pressure(mPAP)on the patients right cardiac catheterization were examinated to analyze the correlation between the dPA,rPA and mPAP.Results After the angiography examination,102 cases were diagnosed as pulmonary hypertension,18 cases with normal pulmonary artery pressure,dPA and rPA in patients with pulmonary hypertension were significantly higher than those with normal pulmonary artery pressure.(P<0.05).Pearson analysis showed that dPA,rPA and mPAP were slightly correlated(P<0.05).Using dPA30 mm as the boundary value,the sensitivity and specificity of the diagnosis of pulmonary hypertension were 90.7%and 66.8%respectively.Using rPA1.0 as the boundary value,the sensitivity and specificity of the diagnosis of pulmonary hypertension was 94.4%and 55.7%respectively.Conclusion The correlation between dPA,rPA and mean pulmonary artery pressure in patients with pulmonary hypertension are significant.The dPA,rPA can be used as an important predictor of pulmonary hypertension and has high clinical diagnostic value.
