交锁髓内钉治疗股骨干骨折的临床观察

摘要 目的:探讨交锁髓内钉治疗股骨干骨折的临床效果和安全性。方法:将108例患者随机分为观察组56例和对照组52例,观察组采用小切口切开复位、交锁髓内钉内固定治疗,对照组采用加压钢板治疗,分析对比两组患者的临床疗效。结果:全部病例均获的6~18个月的随访,平均随访15个月。观察组患者骨折均获得骨性愈合,平均愈合时间为14周,无感染、断钉等并发症;对照组47患者骨折获得骨性愈合,平均愈合时间为20周,5例患者发生感染。二者临床疗效比较差异存在显著性(P<0.05)。结论:采用交锁髓内钉治疗股骨干骨折,操作简单,局部血供破坏小,固定牢固,便于早期功能锻炼,骨折愈合率高,值得临床推广应用。 Objective:To explore the clinical curative effect and safety of interlocking intramedullary nail for treatment of femoral shaft fracture.Methods:108 patients were divided randomly into the intervention group(56 cases)and the control group(52 cases),the intervention group was treated with open reduction of small incision and interlocking intramedullary nail,while the control group was treated with compression plate,and then compared the effect of them.Results:All cases were follow-up for 6~18 months,15 months on average.All cases achieved solid bone union,a mean healing time of 14 weeks,there were no complication in the intervention group;47 cases achieved solid bone union,a mean healing time of 20 weeks,5 cases of infection.That the former were better than the latter(P<0.05).Conclusion:The interlocking intramedullary nail for treatment of femoral shaft fracture,simple Operation,less trauma on regional blood supply,stable and reliable fixation,benefit for early functional exercise,more effective,and hence is worthy of being recommended in clinical practice.
