摘要 目的 :探讨铜绿假单胞菌 (PA)呼吸机相关肺炎 (VAP)的临床特点与防治对策。方法 :对 36例由 PA所致 VAP的患者进行回顾性分析。结果 :PA- VAP的临床特点与其它肺炎相似 ,但病情较重 ,并发症多 ,易发生多器官功能衰竭 (MSOF) ,病死率高。PA对多种抗生素产生耐药 ,而对泰能、舒普深、头孢他啶仍有较高的敏感性。治疗好转率 52 .8% ,病死率 41 .7%。结论 :PA耐药菌株多 ,合理使用抗生素及加强各环节的严格消毒 ,对 PA- VAP的防治十分有益。 Objective:To summarize the clinical characteristics of ventilator associated pneumonia due to Pseudomonas aeruginosa (PA VAP)and its prophylactic therapeutic measures.Methods:The clinical data of 36 patients with PA VAP were retrospectively analyzed.Results: Compared with pneumonia caused by other bacteria,patients with PA VAP were more serious condition,more complications easier to develop multiple system organ failure (MSOF)and higher mortality.Although P.aeruginosa was resistant to many antibiotics,it was sensitive to Tienam,Sulperazon and ceftazidime.The effective rate and mortality in the present study were 52.8% and 41.7%,respectively.Conclusion:Antibiotic resistance in P.aeruginosa is wide.The use of appropriate antibiotics and strictly sterilizing measures may be immensely beneficial for the prophylaxis and treatment of PA VAP.
