苯那普利对家兔动脉粥样硬化形成和斑块破裂的作用 被引量:2

摘要 目的 :观察苯那普利对高胆固醇饲喂的兔动脉粥样硬化形成及斑块破裂的影响。 方法 :34只新西兰雄性大白兔随机分成 4组 ,对照组 (8只 )饲喂普通饲料 ;高脂组 (8只 )饲喂 1%胆固醇饲料 ;苯那普利组 (8只 )饲喂 1%胆固醇饲料及苯那普利 5 m g/ d;斑块破裂组 (10只 ) ,颈动脉球囊扩张损害后喂 1%胆固醇饲料 10周 ,造影证实斑块形成后 ,球囊扩张诱发斑块破裂 ,再随机分苯那普利治疗组和非治疗组 ,继续 1%胆固醇喂养 4周。实验前及实验后 5、10、14周分别抽血测血脂、血管性假血友病因子 (v WF)、循环内皮细胞 (CEC)计数。实验末处死大白兔 ,计算主动脉斑块面积。结果 :苯那普利治疗组较动脉粥样硬化组血脂水平无明显变化 ,动脉粥样硬化斑块面积却明显减少 (P<0 .0 1) ,其血 v WF浓度亦明显降低 (P<0 .0 1)。斑块破裂组球囊扩张术前其血 v WF浓度进行性增高 (P<0 .0 1) ,CEC增高不明显 ,但术后非治疗组 CEC却显著增高 (P<0 .0 1) ,治疗组 CEC、v WF较非治疗组明显减少 (P<0 .0 1,P<0 .0 5 )。结论 :苯那普利具有抗动脉粥样硬化作用。其机制可能与保护内皮功能有关 ;CEC、v WF特别是 CEC对不稳定斑块破裂可能有预测作用 。 Objective: To examine the effect of Benapril on atherogenesis and the plaque rupture in rabbits.Methods: Thirty four healthy male New Zealand white rabbits were randomly divided into 4 groups.Group C (control group) was fed normal diet for 10 weeks,group HL fed 1% cholesterol diet,group B fed 1% cholesterol diet and Benapril 5 mg/d.Ten rabbits of plaque rupture group were fed 1% cholesterol diet for 10 weeks with atherosclerosis induced by left carotid iliac damage.Ten weeks after the initiation of the diet,an angioplasty was performed.After angioplasty,the surviving rabbits( n =10) were randomized to receive benapril(5 mg/d,each) supplementation in drinking(B group, n =4) or no treatment(untreated group, n =4). The levels of CEC were measured by morphometrical counts at different periods(the 5th week,the 10th week).Before and 5,10 weeks after experiment,fasting blood samples were collected for serum total cholesterol(TC),high density lipoprotein cholesterol(HDL C),low density lipoprotein cholesterol(LDL C),and triglyceride(TG) assay.The levels of plasma circulating endothelial cells(CEC) were measured by morphometrical counts and the levels of plasma von Willebrand factor(vWF) were determined by ELISA.After sacrificing the separated, entire aortas were stained with oil red O and then processed for histological examination;planimetry was done with a computer system.Endothelial cells were confined by the presence of Factor Ⅷ related antigen as the specific cell marker.Results: The serum TC,TG,LDL C increased progressively in group B and group HL,but the levels of vWF in group HL were significantly higher than that in group B on the 5th and 10th respetively( P <0.01),and the levels of CEC in each group had no significant difference.The levels of plasma vWF increased progressively before angioplasty( P <0.01)and the levels of CEC had no significant difference.But after angioplasty the levels of CEC were significantly higher than that before angioplasty in untreated group.The levels of plasma vWF and CEC were significantly lower in treated group than that in untreated group.Conclusion: Benapril can inhibit the further progression of atherosclerosis by protecting the endothelial function.The levels of plasma vWF and CEC,especially CEC may be used to predict the unstable plaque rupture of atherogenesis,while Benapril may be able to stablize the plaque.
