不能手术切除的非小细胞肺癌集束电极射频治疗的初步探讨 被引量:1

摘要 目的 :探讨应用集束电极射频 (radiofrequencyablation ;RFA)治疗不能手术切除的非小细胞肺癌的可行性和近期疗效。方法 :在CT引导下经皮肺穿刺或术中直视下RFA治疗不能手术切除的非小细胞肺癌 ,随访观察治疗近期效果、中位生存期、1年生存率、2年生存率及围手术期并发症。结果 :86例患者共 92个肿瘤病灶用RFA治疗 ,术后并发气胸 16例 ,随访5~ 36个月。治疗后 1个月 41 86 %的KPS评分改善。 16例出现局部复发 ,其中 9例患者再次RFA。中位生存时间为 14个月 ,1年生存率为 78 15 %,2年生存率为 46 88%。结论 :RFA治疗不能手术的非小细胞肺癌创伤小 ,可重复治疗 ,安全可靠 ,局部治疗效果较好。 Objective:The aim of this study was to evaluate feasibility and effectiveness of radiofrequency ablation, performed by using a clustered electrode needle, in the treatment of unresectable non-small cell lung cancer.Methods:The lung tumors were treated percutaneously under CT scan guidance or during surgery using a EVB electrode needle and an RF 2 000 generator. All patients are followed to assess treatment response, median survival times, one year survival rate, two year survival rate and perioperative complication.Results:RFA was used to treat 92 tumors in 86 patients. Major postoperative complication was pneumothorax occurred in 16 patients. During 5~36 months follow-up period, the rate of improvement of KPS for patients in the synthetic group was 41.86%. Tumor has recurred in 16 patients. RFA was repeated in 9 patients of recurrence. Median survival times was 14 months. One year survival rate was 78.15%, two year survival rate was 46.88%.Conclusion:RF ablation by EVB electrode needle is a minimally invasive, safe and effective technique for unresectable non-small cell lung cancer.
