摘要
目的 研究挤压伤后多器官功能障碍综合征 (MODS)的发病机制和预防措施。方法 用 32kg重物挤压家兔双后肢大腿和臀部 5h制作挤压伤后MODS的动物模型 ,分别用生理盐水、甘露醇、辅酶Q10 和维生素C于挤压前和解压前静脉给药 ,测定实验动物的器官功能和自由基的变化。结果 3个治疗组解压后 2h内平均动脉压明显高于生理盐水组 (P <0 0 5 ) ,血浆N 乙酰 β D 氨基葡萄糖苷酶 (NAG)、丙二醛 (MDA)和一氧化氮 (NO)含量在解压后 2 4h中亦明显低于生理盐水组 (P <0 0 5 ) ;而组织MDA和NO含量均不同程度的低于生理盐水组 ,且高于正常组 ;3个治疗组的多器官功能障碍 (MODS)的发生率分别为 2 5 0 %、 33 3%、 33 3% ,低于生理盐水组的 60 0 % ,器官组织的损伤程度低于生理盐水组 ;3个治疗组之间各种指标变化差异不大。结论 挤压伤后引起MODS ,其发病机制可能由于缺血再灌注损伤造成 ,在缺血前或 /和再灌注前应用抗自由基的药物 。
Objective To study pathogenesis of multiple organ dysfunction syndrome(MODS)after crush injury and its prevention.Method Animal model of MODS was reproduced in rabbits by crush injury with substance weighed 32 kg on their posterior legs and hips for five hours.Normal saline,mannitol,co-enzyme Q 10 and vitamin C were administered respectively for the four groups of rabbits before and after crush injury,and changes in their organ function,arterial blood pressure and free radical levels were measured. Result Arterial blood pressure was significantly higher in the groups with mannitol,co-enzyme Q 10 and vitamin C,respectively,than that in normal saline group,two hours after relief from crush(P<0.05).Plasma levels of MDA,NAG and nitric oxide(NO)were significantly lower in the three groups than those in the normal saline group within 24 hours after relief from crush(P<0.05).Tissue levels of MDA and NO were lower in the three groups than those in the normal saline group,but higher than those in the normal controls.Incidence of MODS were 25.0%,33.3% and 33.3% in the three groups with mannitol,co-enzyme Q 10 and vitamin C,respectively,lower than 60.0% in normal saline group.Severity of organ dysfunction was less in the three groups with mannitol,co-enzyme Q 10 and vitamin C than that in the normal saline group.No other change was observed in the three groups.Conclusion MODS after crush could be induced by ischemia and reperfusion injury,which could be prevented by free radical scavengers,such as mannitol,co-enzyme Q 10 and vitamin C.
