摘要
目的 :研究母血、脐血和羊水中的瘦素水平与胎儿生长发育的关系 ,探讨瘦素检测的临床价值。方法 :选择5 2例正常产妇 ,根据其新生儿出生体重分为 :大于胎龄儿 14例 ,适于胎龄儿 2 1例 ,小于胎龄儿 17例。采用放射免疫法检测母血、脐血、羊水中的瘦素水平 ,分析瘦素与胎盘重量、新生儿体重、新生儿Ponderal指数 (PI)、孕妇体重、孕妇体重指数 (BMI)、胎儿性别的关系。结果 :①孕妇血清瘦素水平与孕妇体重和体重指数呈正相关。孕妇血清瘦素水平与胎盘重量、新生儿体重、脐血瘦素水平无明显相关性。②大于胎龄儿的脐血瘦素水平高于适于胎龄儿 ,适于胎龄儿又高于小于胎龄儿 ,脐血瘦素水平与新生儿体重、新生儿Ponderal指数、胎盘重量呈正相关。③大于胎龄儿的羊水瘦素水平高于适于胎龄儿 ,适于胎龄儿又高于小于胎龄儿。羊水瘦素水平与新生儿体重、新生儿Ponderal指数、胎盘重量呈正相关。④男婴和女婴脐血和羊水瘦素水平无明显差异。结论 :检测脐血或羊水瘦素水平可以判断胎儿宫内生长发育状况。
Objective:To study the relationship Between leptin concentration and to fetal growth,and to investigate the clinical value of monitoring leptin concentrations. Methods:Leptin concentrations in maternal serum, cord blood and amniotic fluid were measured by radioimmunoassay (RIA) in 52 neonates that included 14 birth weight large for gestational age (LGA), 21 birth weight appropriate for gestational age (AGA), and 17 birth weight small for gestational age(SGA). The correlation among leptin concentrations in maternal serum, cord blood, amniotic fluid and placental weight, birth weight, newborn Ponderal index, maternal weight, maternal body mass index (BMI) and gender were studied. Results:① Maternal serum leptin levels positively correlated with maternal weight (r=0.51) and BMI (r=0.41), but not with cord blood leptin, placental weight and birth weight. ② Higher cord leptin concentrations were detected in LGA infants (15.3±6.6 ng/ml) when compared with AGA infants(9.9±4.9 ng/ml)(P<0.01), and in AGA infants compared to SGA infants(6.0±3.7 ng/ml)(P<0.01). Cord leptin levels positively correlated with birth weight(r=0.58), Ponderal index(r=0.54) and placental weight(r=0.46). Except maternal serum leptin levels. ③ Leptin levels of amniotic fluid in LGA infants (8.6±3.8 ng/ml) and SGA infants (3.6±2.3 ng/ml) were significantly higher and lower, respectively than that in the AGA infants (6.2±3.5 ng/ml). There were significantly positive correlations between amniotic fluid leptin levels and birth weight(r=0.57), Ponderal index(r=0.50), and placental weight (r=0.44). Amniotic fluid leptin concentration was strongly related to cord blood leptin concentration(r=0.45) but not maternal serum leptin concentration. ④ Leptin levels in cord blood and amniotic fluid were no difference between male and female neonates. Conclusions:Leptin levels in cord blood or amniotic fluid can be used as a marker for monitoning fetal growth and development.
