摘要
目的探讨血清中嗜酸性粒细胞趋化因子(Eotaxin)、嗜酸性阳离子蛋白(ECP)、白细胞介素-4(IL-4)、γ-干扰素(IFN-γ)、外周血中Treg细胞的变化与呼吸道合胞病毒(RSV)毛细支气管炎患儿病情、喘息复发的关系。方法选取青岛市中心医院90例RSV毛细支气管炎患儿(RSV组)、90名健康体检儿童(对照组),检测两组血清Eotaxin、ECP、IFN-γ、IL-4及外周血中Treg细胞,根据患儿病情、喘息复发情况进行分层分析。结果 RSV组患儿治疗前血清Eotaxin、ECP、IL-4检测值均显著高于对照组(P<0.05),RSV组患儿治疗前血清IFN-γ外周血中Treg细胞检测值均显著低于对照组(P<0.05);重症组RSV患儿治疗前血清Eotaxin、ECP、IL-4检测值均显著高于轻症组(P<0.05),重症组RSV患儿治疗前血清IFN-γ外周血中Treg细胞检测值均显著低于轻症组(P<0.05)。治疗1周后,RSV患儿的血清Eotaxin、ECP、IL-4检测值较治疗前明显降低(P<0.05),血清IFN-γ外周血中Treg细胞检测值呈不同程度升高(P<0.05)。结论 RSV毛细支气管炎患儿存在明显的免疫失衡,检测血清Eotaxin、ECP、IFN-γ、IL-4及外周血中Treg细胞对指导临床治疗具有一定的价值。
Objective To investigate the changes of eotaxin,eosinophilic cationic protein(ECP),interleukin-4(IL-4),γ-interferon(IFN-γ)and Treg cells in peripheral blood,and their Relationship with wheezing recurrence in children with respiratory syncytial virus(RSV)bronchiolitis.Methods 90 children with RSV bronchiolitis(RSV group)and 90 healthy children(control group)were enrolled in our hospital.Serum Eotaxin,ECP,IFN-γ,IL-4 and Treg cells in peripheral blood were detected in the two groups.According to the condition of the child and the recurrence of wheezing,a stratified analysis was performed.Results The serum levels of Eotaxin,ECP and IL-4 in the RSV group were significantly higher than those in the control group(P<0.05).The serum levels of IFN-γin the RSV group were significantly higher than those in thecontrol group.The levels of serum Eotaxin,ECP,and IL-4 in the severe group of children with RSV were significantly higher than those in the mild group(P<0.05),and those in the RSV group before treatment.The detection value of Treg cells in peripheral blood of the severe group was significantly lower than that of the mild group(P<0.05).After 1 week of treatment,the serum Eotaxin,ECP and IL-4 levels in children with RSV were significantly lower than those before treatment(P<0.05).The detection value of Treg cells in peripheral blood increased to different degrees(P<0.05).Conclusion There was a significant immune imbalance in children with RSV bronchiolitis.The detection of serum Eotaxin,ECP,IFN-γ,IL-4 and Treg cells in peripheral blood had certain clinical value for children with RSV and could be used to judge and guide clinical treatment.
