miR-18a表达在HPV感染及宫颈癌发生发展中的意义 被引量:9

摘要 目的研究宫颈脱落细胞中miR-18a的表达水平在人乳头状瘤病毒(HPV)感染及宫颈癌发生发展中的意义。方法收集2015年6月至2017年6月深圳市南山区妇幼保健院及广州南方医院门诊及住院患者宫颈脱落细胞标本,通过HPV DNA分型检测将宫颈脱落细胞标本分为HPV未感染组(NE-HPV)、低危型HPV感染组(LR-HPV)、高危型HPV感染组(HR-HPV),采用荧光定量PCR法(qRT-PCR)检测各组间miR-18a相对表达量;收集HR-HPV感染阳性的宫颈脱落细胞标本,依据病理检查结果分为宫颈上皮内瘤变(CIN)组及宫颈癌组各30例,对照组选取年龄、体重配对的30例宫颈上皮细胞正常无化生的标本,采用qRT-PCR检测各组间miR-18a相对表达量,分析miR-18a表达与HPV感染及宫颈癌发生发展的关系。结果 HR-HPV感染组宫颈脱落细胞中miR-18a相对表达量高于LR-HPV组与NE-HPV组,差异均有统计学意义(t值分别为11.144、18.482,均P<0.05);宫颈癌组、CIN组中miR-18a表达较对照组显著增高(t值分别为25.02、15.355,均P<0.05),但宫颈癌组与CIN组之间比较差异无统计学意义(P>0.05)。ROC曲线显示宫颈脱落细胞中miR-18a用于诊断CIN的曲线下面积(AUC)为0.699(95%CI:0.606~0.791,P<0.001),用于诊断宫颈癌的AUC为0.801(95%CI:0.724~0.879,P<0.001)。结论 HR-HPV感染者宫颈脱落细胞中miR-18a相对表达量明显升高,并与宫颈癌的发生发展密切相关。宫颈脱落细胞中miR-18a可能作为宫颈癌或癌前病变潜在标志物用于早期无创诊断。 Objective To study the expression of miR-18 a in cervical exfoliated cells and its significance in HPV infection and cervical cancer development. Methods Cervical cytological specimens were divided into negative HPV group(NE-HPV), low-risk group(LR-HPV) and high-risk group(HR-HPV) by HPV DNA genotyping. The levels of miR-18 a expression of each group were determined by fluorescent quantitative PCR. Cervical exfoliated cell samples infected with HR-HPV were divided into cervical intraepithelial neoplasia(CIN) group and cervical cancer group with 30 cases by pathological examination. Control group was samples of 30 cases of normal cervical cells paired in age and weight. QRT-PCR was used to compare the expression levels of miR-18 a among three groups. The relationship between miR-18 a expression and HPV infection and cervical cancer was analyzed. Results The expression of miR-18 a in HR-HPV group was significantly higher than in LR-HPV group and un-infected group(t value was 11.144 and 18.482, respectively, both P<0.01). MiR-18 a expression in cervical cancer group and CIN group was significantly higher than that in the control group(t value was 25.02 and 15.355, respectively, both P<0.05). However, there was no significant difference between cervical cancer group and CIN group(P>0.05). The AUC value under ROC curve of miR-18 a expression in cervical exfoliated cells for diagnosis of CIN was 0. 699(95% CI: 0.606-0.791,P<0.001), and the AUC value for diagnosis of cervical cancer was 0.801(95% CI: 0.724-0.879,P<0.001). Conclusion The relative expression of miR-18 a in cervical exfoliated cells infected with HR-HPV is significantly increased, which is closely related to the occurrence and development of cervical cancer. MiR-18 a in cervical exfoliated cells may be used as a potential marker of cervical cancer or precancerous lesions for early non-invasive diagnosis.
