摘要
目的研究自制锚钉在颈后路单开门椎管扩大椎板成形术(ELAP)中的应用效果。方法 47例纳入研究的病例采用自制锚钉进行固定,分析病例临床资料,统计数据,并应用SPSS17.0软件分析。结果 47例自制锚钉置入过程顺利,未出现脊髓、椎动脉及神经损伤,手术时间70~140(94.2±8.3)分钟,出血量160~600(263.4±65.6)m L。术中未输血,术后未出现感染及异物排异反应。切口均为1期愈合。术后随访过程颈椎X片及CT显示'开门'程度满意,未见锚钉松动、脱落以及颈椎不稳、椎板塌陷现象。18个月行脊髓功能JOA评分,结果与治疗前JOA评分比较有显著性差异(<0.05)。结论自制锚钉应在颈后路ELAP术中操作简单,安全性可靠,临床'开门'效果满意。
Objective To investigate the application effect of homemade anchors in posterior spinal expand open-door laminoplasty. Methods 47 cases included in the study were fixed by homemade anchor. All the cases of clinical data were been analysed and done the statistics by analysis application software SPSS17. Results 47 cases of homemade anchors were placed smoothly, without spinal cord, vertebral artery and nerve injury, the operation time were 70 to 140(94.2±8.3)min, bleeding were 160 to 600(263.4±65.6) m L. No blood transfusion intraoperative, Both infection and foreign body rejection does not appear postoperative. Incisions were all I healing, cervical X-ray and CT showed satisfactory 'open' degree in postoperative follow-up process, and no anchor loose, loss, and cervical instability, laminectomy collapse phenomenon. Spinal cord function JOA score were taken the first 18 months, JOA score had statistically significant compared with before treatment( <0.05). Conclusion Homemade anchors used in intraoperative posterior ELAP simple,security reliable, the effect of clinical 'open the door' was satisfaction.
