摘要
目的探讨超声检查对脑梗死患者颈动脉粥样硬化的诊断价值。方法选取2014年9月—2015年11月荆州市第一人民医院收治的脑梗死患者85例为观察组,同期在本院体检的健康人85例为对照组。两组受试者均进行颈动脉超声检查,观察其颈总动脉内膜-中层厚度(IMT)、颈总动脉血流指标、颈动脉斑块类型及颈动脉狭窄程度。结果观察组患者左、右侧颈总动脉IMT高于对照组,颈总动脉最大血流速度(Vmax)、最小血流速度(Vmin)、左颈总动脉收缩期峰值流速(SPV)、舒张末期容积(EDV)低于对照组,阻力指数(RI)、颈总动脉(CCA)厚度及颈动脉分叉处(BIF)厚度、搏动指数(PI)高于对照组(P<0.05)。两组受试者颈动脉斑块类型比较,差异有统计学意义(P<0.05);观察组患者颈动脉狭窄程度重于对照组(P<0.05)。结论脑梗死患者颈动脉粥样硬化程度较重,表现为颈总动脉IMT增厚、颈总动脉血流变化,超声检查具有很好的诊断价值。
Objective To explore the diagnostic value of ultrasonography on carotid atherosclerosis of patients with brain infarction. Methods From September 2014 to November 2015 in the First People ’s Hospital of Jingzhou,a total of 85 patients with brain infarction were selected as observation group,85 healthy cases were selected as control group. Subjects of the two groups received ultrasonography after admission,including IMT and blood flow index of common carotid artery,carotid artery plaques types and degree of carotid artery stenosis. Results Left and right IMT of common carotid artery of observation group were statistically significant higher than those of control group, Vmax, Vmin, SPV and EDV of observation group were statistically significant lower than those of control group,while RI,CCA thickness,BIF thickness and PI of observation group were statistically significant higher than those of control group( P < 0. 05). There was statistically significant differences of carotid artery plaques types between the two groups( P < 0. 05), and degree of carotid stenosis of observation group was statistically significant more severe than that of control group( P < 0. 05). Conclusion Carotid atherosclerosis of patients with brain infarction is relatively severe,most of them performed as increased IMT of common carotid artery and change of carotid artery blood flow,ultrasonography has good diagnostic value on carotid atherosclerosis of patients with brain infarction.
