摘要
目的比较超声检查与宫腔镜检查在绝经后子宫出血中的应用效果。方法选取2016年成都市金堂县第一人民医院收治的绝经后子宫出血患者200例。所有患者分别采取超声检查和宫腔镜检查,并行宫腔镜直视下活检或分段诊断性刮宫病理检查。观察所有患者病理学检查结果,比较超声检查与宫腔镜检查患者的灵敏度、特异度、阳性预测值、阴性预测值、诊断符合率。结果 200例患者中子宫内膜增生症65例,子宫内膜炎33例,子宫内膜息肉30例,子宫黏膜下肌瘤24例,子宫内膜癌21例,正常增生期及分泌期子宫内膜18例,子宫颈管息肉5例,子宫颈癌4例。超声检查与宫腔镜检查患者的灵敏度比较,差异无统计学意义(P>0.05);超声检查患者的特异度、阳性预测价值、阴性预测价值、诊断符合率低于宫腔镜检查患者(P<0.05)。结论与超声检查相比,宫腔镜检查在绝经后子宫出血中的应用效果较好。
Objective To compare the effect of ultrasonography and hysteroscopy in uterine hemorrhage after menopause. Methods In 2016,a total of 200 patients with uterine hemorrhage after menopause were selected in the Chengdu Jintang County First People' s Hospital. All patients underwent ultrasonography and hysteroscopy,and the parallel hysteroscopy was performed in the diagnosis of biopsy or subsection diagnosis. The pathological examination results of all patients were observed,and the sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic compliance rate of patients with ultrasonography were compared. Results In patients with 200 cases, of the 65 patients were endometrial hyperplasia,of the 33 patients were endometritis,of the 30 patients were endometrial polyp,of the 24 patients were submucosal myoma of uterus, of the 21 patients were endometrial carcinoma, of the 18 patennts were normal hyperplasia and secretory endometrium,of the 5 patients were cervical polyp,of the 4 patients were cervical cancer. Sensitivity no significant differences in the ultrasonography and hysteroscopy with uterine hemorrhage after menopause( P > 0. 05); specificity, positive predictive value,negative predictive value, diagnostic coincidence rate of the ultrasonography with uterine hemorrhage after menopause were lower than those of hysteroscopy with uterine hemorrhage after menopause( P < 0. 05). Conclusion Compared with ultrasonography,hysteroscopy is effective in postmenopausal uterine bleeding.
