64排螺旋CT不同重建图像对鼻骨骨折的诊断价值 被引量:7

摘要 目的比较64排螺旋CT横断面、冠状面、三维容积再现图像(VR)对鼻骨骨折的诊断价值。方法连续收集行鼻骨CT扫描的鼻外伤患者95例。采用横轴位螺旋扫描,120 kV,200 mA,层厚2.5 mm,螺距0.531重建横断面、冠状面、VR图像。由2名高年资医师分别对同一患者的单一横断面、冠状面、VR图像及2种图像组合、3种图像组合作出诊断。采用多组等级资料比较的秩和检验,比较诊断结果差异。结果各种图像诊断结果差异无统计学意义(P>0.05)。结论虽然本组资料显示不同单种图像、组合图像在诊断鼻骨是否骨折和骨折类型方面的差异无统计学意义,但是,为避免漏诊、误诊、过诊,采用横断面加VR图像,既方便又效价比高。 Objective To evaluate the diagnostic value of the various reconstructing images of MDCT of nasal bone. Methods Ninety-five cases whose nasal region injured underwent MDCT. Their axial images, coronal images, VR-images of nasal bones were obtained. Two veteran radiologists independently reviewed the each type of image of each patient and made diagnosis about the injured nasal bone by one kind of these images or paired images. The data was analyzed with K Independent Samples tests of Nonparametric Tests of SPSS 13.0. Results There was no signifi-cant difference among the different types of images and the different types of their combinations. Conclusion The study shows there is no significant difference basing on the different types of images to determine the nasal bone frac-ture and the fracture classification. However, to avoid that the nasal bone fracture could be misdiagnosed, missed or excessively diagnosed, the combination of the axial image and the VR-image should be used.
