乳腺癌18F-FDG PET/CT代谢参数与TNM分期及Ki-67表达的关系 被引量:10

Relationship between 18F-FDG PET/CT metabolic parameters and TNM stages and Ki-67 expression in breast cancer patients

摘要 目的探讨乳腺癌治疗前18F-FDG PET/CT显像中的3种代谢参数最大标准摄取值(maximum standardized uptake value,SUVmax)、肿瘤代谢体积(metabolic tumor volume,MTV)以及糖酵解总量(total lesion glycolysis,TLG)与肿瘤TNM分期及增殖细胞核抗原Ki-67的关系。方法回顾性分析52例乳腺癌患者治疗前的18F-FDG PET/CT显像。在影像工作站手动勾画病灶感兴趣区(region of interest,ROI),提取病灶SUVmax、MTV和TLG。采用Mann-Whitney秩和检验方法,分析3种PET/CT显像的代谢参数与TNM分期及Ki-67之间的关系。结果SUVmax、MTV和TLG这3种代谢参数在T分期T0~1组和T2~4组间比较,差异均具有统计学意义(均P<0.05)。MTV和TLG在N分期N0组和N1~3组间比较,差异均具有统计学意义(均P<0.05),而SUVmax在两组间比较,差异无统计学意义(P>0.05)。3种代谢参数在M分期M0组和M1组间比较,差异均无统计学意义(均P>0.05)。以20%作为Ki-67高低表达的阈值,3种代谢参数在Ki-67高表达组和低表达组间比较,差异均具有统计学意义(均P<0.05)。结论 18F-FDG PET/CT代谢参数MTV和TLG可以作为评估乳腺癌治疗前T、N分期及Ki-67指标的临床评价参数。 Objective To explore the relationship between preoperative 18F-fluorodeoxyglucose(18F-FDG)PET/CT metabolic parameters[maximum standardized uptake value(SUVmax),metabolic tumor volume(MTV)and total lesion glycolysis(TLG)]and TNM staging and proliferating cell nuclear antigen Ki-67 expression in breast cancer patients.Methods Fifty-two preoperative 18F-FDG PET/CT images of breast cancer patients were retrospectively analyzed.The SUVmax,MTV and TLG of primary breast cancer were measured and extracted by region of interest(ROI)technique on imaging workstation.The metabolic parameters were compared with TNM staging and Ki-67 expression by rank sum test of Mann-Whitney.Results The three metabolic parameters of SUVmax,MTV and TLG were compared between the T staging subgroups(T0-1 group and T2-4 group),and the differences were statistically significant(P<0.05).MTV and TLG were significantly different in N staging subgroups(N0 group and N1-3 group,both P<0.05),while SUVmax was not significantly different between the two groups(P>0.05).The three metabolic parameters were compared in M staging subgroups,and the differences were not statistically significant(all P>0.05).When 20%was determined as the threshold for the expression of Ki-67,the three metabolic parameters were statistically different between the Ki-67 high-expression and Ki-67 low-expression subgroups(all P<0.05).Conclusions The metabolic parameters MTV and TLG of preoperative 18F-FDG PET/CT can be clinically used as the parameters to assess T and N staging and Ki-67 expression in breast cancer patients.
