摘要
目的分析低分子肝素预防下肢静脉曲张术后深静脉血栓形成的效果。方法选取该院2016年6月—2018年1月收治的下肢静脉曲张患者80例,将其随机分为2组,各40例。两组均行大隐静脉高位结扎+腔内激光闭合术+分段剥脱术治疗,术后观察组采用低分子肝素预防深静脉血栓形成,而对照组不使用抗凝药物。比较两组术后并发症、深静脉形成等发生率、随访神经损伤和疾病复发情况以及生存质量。结果在并发症方面,两组肝功能异常、血小板减少、切口出血、浸透敷料出血等发生率对比差异无统计学意义(P>0.05);而在术后深静脉血栓形成方面,观察组发生率2.5%较对照组15.0%明显较低,差异有统计学意义(P<0.05)。经随访,观察组在神经损伤发生率和下肢静脉曲张复发率上,观察组较对照组均显著较少,且P<0.05差异有统计学意义。在生存质量上,观察组各项评分较对照组均显著较高,差异有统计学意义(P<0.05)。结论低分子肝素预防下肢静脉曲张术后深静脉血栓形成的效果非常显著,且可降低神经损伤,预防疾病复发,因为安全性较高,值得临床应用推广。
Objective To analyze the effect of low molecular weight heparin(LMWH) in preventing deep venous thrombosis after varicose veins surgery in lower extremities. Methods 80 patients with varicose veins of lower extremities admitted to our hospital from June 2016 to January 2018 were randomly divided into two groups, 40 cases each. Both groups were treated with high ligation of great saphenous vein, intravenous laser closure and segmental stripping. The observation group was treated with low molecular weight heparin to prevent deep venous thrombosis, while the control group was not treated with anticoagulant drugs. The incidence of complications, deep vein formation, nerve injury, recurrence and quality of life were compared between the two groups. Results In terms of complications, there was no significant difference in the incidence of liver dysfunction, thrombocytopenia, incision hemorrhage and immersion dressing hemorrhage between the two groups, and there was no statistical significance(P>0.05);in terms of deep vein thrombosis after operation, the incidence of 2.5% in the observation group was significantly lower than that of 15.0% in the control group, and the difference was statistically significant(P<0.05). After follow-up, the incidence of nerve injury and the recurrence rate of varicose veins in the observation group were significantly less than those in the control group, and the difference was statistically significant(P<0.05). In terms of quality of life, the scores of the observation group were significantly higher than those of the control group, and the difference was statistically significant(P <0.05). Conclusion Low molecular weight heparin can prevent deep venous thrombosis after varicose veins surgery of lower extremities, reduce nerve injury and prevent recurrence of disease, because of its high safety, it is worthy of clinical application and promotion.
