摘要
目的 :探讨检测T细胞受体γ(TCR γ)基因重排的方法用于蕈样肉芽肿 (MF)早期辅助诊断。方法 :采用“一步法”提取 10 %福尔马林固定、石蜡包埋组织的DNA作为模板 ,聚合酶链反应 /琼脂糖凝胶电泳方法 ,检测4 6例蕈样肉芽肿 ,19例可疑蕈样肉芽肿 ,10例湿疹皮炎 ,5例类银屑病及 5例正常皮肤石蜡包埋组织的TCR γ(V1~V8和V9)基因重排。结果 :与非MF病人相比 ,81%的二期蕈样肉芽肿病人显示TCR γ基因重排 ,而非MF病人未见TCR γ基因重排。结论 :“一步法”提取的石蜡包埋组织的DNA作为模板的PCR技术可用于检测MF石蜡包埋组织的TCR γ基因重排 ,且是一种灵敏度和特异性均较高的方法 。
Objective: Our aim was to detect the rearrangement of T cell receptor gamma(TCR-γ) gene for the early diagnosis of mycosis fungoides(MF). Methods:We used one-step method to extract DNA from formalin-fixed(10%) and paraffin-embedded tissue blocks. Forty-six cases of mycosis fungoides(MF), 19 suspected cases of MF, 10 of eczema dermatitis, 5 of parapsoriasis, and 5 of normal skin were included in this study. The TCR-gamma gene rearrangement was detected with polymerase chain reaction and agrose electrophorcsis. Results:TCR-gamma gene rearrangement was detected in 34 of MF cases(81%) while no earrangement in other cases. Conclusion:The DNA extracted from paraffin-embedded specimens with one-step method can be used as template to detect TCR-γ rearrangement with PCR. This method is highly sensitive and specific for the early diagnostic of MF.
