深Ⅱ度烧伤创面早期磨痂后大鼠血中LPS、TNF-α及IL-6的变化 被引量:17

摘要 目的:通过检测烧伤大鼠血浆中LPS、血清TNF-α和IL-6浓度的变化,观察大面积深Ⅱ度创面早期磨痂术对机体的影响。方法:将120只30%TBSA深Ⅱ度烫伤大鼠,随机均分为磨痂组(A组)和非手术组(B组),于伤后不同时相点检测血浆LPS、血清TNF-α及IL-6浓度。另取6只正常大鼠(C组)作为对照。结果:A、B两组伤后3h血浆LPS水平开始升高,分别于12h、6d同时出现两个高峰,A组第一个峰值持续时间较长,第二个峰值水平明显低于B组(P<0.05)。伤后24h~3d,两组血清TNF-α水平无明显差别,4~7d,A组水平明显低于B组(P<0.05)。伤后48h~7d,A组血清IL-6浓度均明显低于B组(P<0.05)。结论:休克期尽早对大面积深Ⅱ度创面进行磨痂手术对机体是安全的,且有利于减轻全身炎症反应综合征。 Objective:To investigate the concentration changes of circulating lipopolysaccharide(LPS),tumor necrosis factor alpha(TNF-α)and interleukin-6(IL-6)in scalded rats,so as to study the influ-ence of woundabasion in shock stage.Methods:One handred and twenty male Wister rats with30%TB-SA deep second degree scalding were randomly divided into woundabrasion group(group A)and no-opera-tion group(group B),another6normal rats were included into control group(group C).Plasma concentra-tion of LPS was determined by limulus amebocyte chromogenic assay test.Serum concentrations of TNF-αand IL-6were measured by radioimmunoassay.Results:The concentration of plasma LPS increased evident-ly from3PBH in group A and B and reached peak levels at12PBH and6PBD respectively.The first peak level in group A lasted for a longer period than that in group B,and there was no obvious differ-ence in the first peak levels between the two guoups.But the second peak level in group A decreased more evidently than that in group B.There was no difference in the increased TNF-αconcentration from24PBH to3PBD betweem the two groups.However,it was significantly lower in group A than that in group B from4PBD to7PBD.Serum IL-6level in group A was lower than that in group B from48PBH to7PBD.Conclu sion:Woundabrasion in shock stage is feasible and safty to the body by alleviating the systmetic inflammatory response syndrome.
