联合干预对2型糖尿病病人抑郁症状及糖代谢的影响 被引量:34

摘要 目的探讨 2型糖尿病病人的抑郁症状及其联合 (糖尿病教育和心理干预 )干预效果。方法对 96例 2型糖尿病和 6 3例健康人 (对照 1组 )进行首次抑郁自评量表 (SDS)和Hamilton抑郁量表 (HAMD)测评。将 96例 2型糖尿病随机分为观察组和对照 2组 ,对照 2组 4 8例行常规治疗和护理 ,观察组 4 8例在常规治疗的基础上行联合干预。干预 3个月末再行SDS及HAMD测评 ,同时于干预前及干预 3个月末行糖代谢指标检测。结果干预前SDS及HAMD评分观察组明显高于对照 1组 (P <0 .0 1) ,与对照 2组比较 ,差异无显著性意义 (P >0 .0 5 ) ;干预 3个月后 ,观察组SDS及HAMD评分明显低于对照 2组 (P <0 .0 1) ,抑郁症状发生率 (35 .4 % )显著低于对照 2组(5 8.3% ) (P <0 .0 5 ) ;观察组FBS、2hBS及HbAIC 3项指标的改善均明显优于对照 2组 (均P <0 .0 5 )。结论 2型糖尿病病人有较高的抑郁症状发生率 ,联合干预可改善 2型糖尿病病人的抑郁症状及糖代谢。 Objective To explore the depression of diabetes patients and to evaluate the effect of the systemic education and psychology interference. Methods 96 cases of type 2 diabetes mellitus and 63 normal healthy men (control group 1) were assessed by SDS and HAMD. 96 patients with type 2 diabetes mellitus were randomly divided into two groups: 48 cases (control group 2) received the routine treatment and the remaining 48 cases (observation group) were subtected to the systemic education and psychology interference in addition to the routine treatment. SDS, HAMD were adopted at the end of 3 months after the interference. At the same time, carbohydrate metabolism was monitored before and at the end of 3 months after the interference. Results Before the interference, the scores of the SDS and HAMD in the observation group were higher than in control group 1 (P<0.01), but there was no significant difference in comparison with control group 2 (P>0.05). Three months after the systemic education and interference, SDS and HAMD scores and incidence of depression in the observation group were significantly lower than in the control group 2(P<0.01 or P<0.05). In addition, FBS, 2hBS and HbAIC in the observation group were obviously improved as compared with those in control group (all P<0.01). Conclusion The incidence of depression was higher in the Patieats with type 2 diabetes. The systemic education and psychology interference could improve the depression and carbohydrate metabolism.
