尿激酶对前部缺血性视神经病变患眼的血动力学影响 被引量:2

摘要 目的 探讨尿激酶治疗对前部缺血性视神经病变 (anterior ischem ic optic neuropathy,AION)患眼血流动力学的影响。方法 应用彩色多普勒血流成像技术 (color Doppler flow imaging,CDFI) ,观察尿激酶治疗前部缺血性视神经病变 32例 (38只眼 )眼睫状后动脉 (posterior ciliary arteries,PCAs)、眼动脉 (ophthalmic arteries,OA )血流动力学的变化、并与对照组疗效进行比较。结果 尿激酶治疗组中治疗前后 PCAs的收缩期峰值速度(peak systolic velocity,PSV )、舒张末期速度 (end- diastolic velocity,EDV)分别由 (6 .10± 1.5 5 ) cm/ s、(1.96± 0 .19)cm / s增加到 (13.49± 1.5 8) cm/ s、(6 .0 5± 0 .71) cm / s,阻力指数 (resistance index,RI)由 0 .6 7± 0 .0 1降低至 0 .5 6±0 .0 3;尿激酶治疗组中治疗前后 OA的 PSV、EDV平均值分别由 (2 8.87± 3.11) cm / s、(12 .73± 1.38) cm / s增加到(34 .0 8± 2 .88) cm / s、(19.95± 2 .38) cm / s,阻力指数由 0 .72± 0 .0 2降低至 0 .6 5± 0 .0 2 (P <0 .0 0 1)。同对照组比较 ,尿激酶治疗组视功能改善率明显提高 (χ2 =2 8.12 ,P <0 .0 0 1)。结论 尿激酶治疗可以改善 AION的血流动力学参数 ,对探寻一种简便、有效的治疗方法有着积极的意义? Objective To study the effection of Urokinase on retrobulbar hemodynamics in anterior ischemic optic neuropathy,Methods 38 eyes of 32 patients with anterior ischemic optic neuropathy clinically diagnosed were studied by color Doppler flow imaging (CDFI).The peak systolic velocity(PSV),end-diastolic velocity (EDV),and resistance index (RI) of the posterior ciliary arteries (PCAs) and ophthalmic artery (OA) were detected before and after the veinal administration of Urokinase.36 eyes of 30 patients with anterior ischemic optic neuropathy were served as the control group.Results PSV,EDV in PCAs after administraction of Urokiese increseaed from(6.10±1.55)cm/s,(1.96±0.19)cm/s to (13.49±1.58)cm/s,(6.05±0.71)cm/s respectively,RI in PCAs after administration of Urokinase decreseaed from (0.67±0.01) to (0.56±0.03);PSV,EDV and in OA after administration of Urokiese increseaed from (28.87±3.11)cm/s,(12.73±1.38)cm/s to (34.08±2.88)cm/s,(19.95±2.38)cm/s respectiely,RI in OA after administration of Urokiese decreseaed from 0.72±0.02 to 0.65±0.02(P<0.001 respectively).In the comparison with the control group,much recovery of visual function regained in the patients of administration of Urokinase (χ 2=28.12,P<0.001).Conclusions Urokinase could incresead the blood flow of retrobulbar arteries of the AION,is helpful to the treatment of AION.
