摘要
为探讨科雷氏骨折各种固定体位的生物力学机制 ,寻求最佳的固定体位 ,采用三维空间有限元计算方法 ,在计算机内建立完整的前臂模型 ,将前臂和腕部的主要肌肉和腕掌、背侧韧带加载于模型之上 ,分析腕关节在背伸、中立、掌屈位以及尺偏、桡偏等多种工况下 ,应力、应变和位移的变化特点 ,结果显示当腕关节中立、稍尺偏、前臂旋后位时 ,骨折断面的掌、背、尺、桡侧中点的应力均为压应力 ,且掌、背侧数值比几乎为 1∶ 1 ,骨折断面掌、背、尺、桡侧中点的剪切位移量也最小 ,说明前臂旋后腕中立稍尺偏的固定体位最好 ,是治疗科雷氏骨折的最佳固定方式。
To study the biomechanical mechanism of different fixation postures of Colles fracture and to find the best fixation posture,the three-dimensional-finite-element forearm models with main muscles and ligaments were used to analyze the changes of stress,strain and displacement under angulated wrist,such as dorsiflexes,neutrality,palmarflexes,ulnar or radial deviation.The results showed that the stress of Colles fracture all was pressure strain and almost as large as each other at forearm supination,and the displacement of fracture face was the smallest under the neutrality and slightly-ulnar-deviation wrist.It indicates that the forearm supination and wrist slightly-ulnar-deviation neutrality postures are the best fixation postures for Colles fracture.
