摘要
目的 探讨老年阻塞性睡眠呼吸暂停综合征 (OSAS)患者血管内皮功能变化与冠心病的相关性。 方法 选择中、重度OSAS患者 4 7例为OSAS组 ,又分伴冠心病 (18例 )和不伴冠心病(2 9例 )两亚组 ;另选 17例单纯冠心病患者和健康老年人 31例作为对照组。比较各组间血浆一氧化氮 (NO)、内皮素 (ET)和NO/ET的动态变化。 结果 与对照组比较 ,OSAS两组及单纯冠心病组患者NO水平降低 ,ET水平增高 ,NO/ET比值下降 (均为P <0 0 5 ) ,而OSAS伴冠心病者下降更明显。OSAS不伴冠心病者的NO、ET水平及NO/ET比值与单纯冠心病组比较 ,差异无显著性 ,OSAS伴冠心病者的NO和ET水平亦无显著性差异 ,但NO/ET值下降已有统计学意义 (P <0 0 5 )。 结论 OSAS老年患者存在明显血管内皮功能障碍 ,尤以伴冠心病者为甚 。
Objective To investigate the association between function changes of vascular endothelium and coronary heart disease (CHD) in elderly patients with obstructive sleep apnea syndrome (OSAS). Methods Forty-seven elderly patients with moderate or severe degree of OSAS were recruited as OSAS group, which were further divided into CHD subgroup(18 patients)and non-CHD subgroup (29 patients ). Seventeen elderly patients with only CHD were selected as simple CHD group and 31 healthy elderly people as control group. The changes of plasma concentrations of nitric oxide (NO) and endothelin ( ET ) and NO/ET were tested and compared among groups. Results Compared with control group, there was a significant lower NO level, higher ET level and lower NO/ET in two OSAS subgroups and simple CHD group (all P<0.05). Such a difference was especially significant between control group and OSAS with CHD subgroup. The NO and ET levels in simple CHD group showed no significantly different from OSAS with non-CHD subgroup or from OSAS with CHD subgroup as well. However, the NO/ET rate was significantly declined in OSAS with CHD group than in simple CHD group. Conclusions Vascular endothelial function is significantly impaired in elderly patients with OSAS, especially in those with both OSAS and CHD. Vascular endothelial dysfunction may be one of the causes of complicated CHD in elderly OSAS patients.
