摘要
目的:通过回顾性研究分析输尿管镜碎石取石术后放置双J管患者发生全身炎症反应综合征(SIRS)的风险因素,为临床预防SIRS发生提供有效证据。方法:收集2014年1月~2015年1月云南省第一人民医院泌尿外科输尿管镜碎石取石术后放置双J管患者临床资料,根据术后是否发生SIRS将患者分成两组,通过单因素和多因素Logistic回归分析探索术后发生SIRS的风险因素。结果:共收集192例患者(男125例,女67例)临床资料,5.7%(11/192)术后出现SIRS征象;单因素分析发现,诊断有感染(P=0.043)、肾畸形(P=0.008)、血肌酐水平(P=0.006)在两组患者中存在显著差异;多因素回顾分析发现肾畸形是术后发生SIRS的独立风险因素(P=0.025)。结论:肾畸形增加输尿管软镜碎石术后放置双J管患者发生SIRS的风险,术前应予筛查,以预防SIRS所致的脓毒血症的发生。
Objective:To retrospectively study the risk factors of systemic inflammatory response syndrome(SIRS)in patients with double J tube after ureteroscopic lithotripsy in order to provide effective evidence for clinical prevention of SIRS.Methods:A data collection form designed was used to collect the clinical data of patients with double J tube after ureteroscopic lithotripsy in Department of Urology of the First People’s Hospital of Yunnan Province from January 2014 to January 2015,and patients were divided into two groups according to whether SIRS occurred after surgery,then univariate and multivariate logistic regression analyses were conducted to explore the risk factors of SIRS.Results:A total of 192 patients(125 males and 67 females)were included,and5.7%(11/192)patients had SIRS appearance.Univariate analysis indicated that infection(P=0.043),renal malformation(P=0.008)and serum creatinine levels(P=0.006)were significantly different between two groups.Multivariate retrospective analysis revaled that renal malformation(P=0.025)was an independent risk factor of SIRS.Conclusions:Renal malformation increases the risk of SIRS in patients with double J tube after ureteroscopic lithotripsy,and reoperative examination should be performed to prevent the occurrence of sepsis caused by SIRS.
