摘要
目的 从生物力学的角度探讨无支架四叶牛心包二尖瓣交界缝合缘的合适宽度。方法 把经戊二醛鞣制后的牛心包试片测厚、切断后按距切断缘不同宽度缝合 ,用软组织生物材料实验机进行单向静载拉伸试验 ,测定其最大负荷、极限抗张强度及断裂延伸率。根据不同的缝合缘宽度分为四组 ,即 1mm宽缝合缘组、2mm宽缝合缘组、3mm宽缝合缘组、4mm宽缝合缘组。结果 2mm、3mm及 4mm宽缝合缘组试片的平均最大负荷分别为 (10 .5 6± 1.95 )N、(13.5 7± 3.2 7)N和 (15 .71± 3.86 )N ;平均极限抗张强度分别为 (6 .88± 1.2 9)mPa、(8.2 5± 2 .5 8)mPa和 (8.94± 2 .0 5 )mPa ;断裂延伸率 (2 9.95± 4 .11) %、(33.31± 5 .0 2 ) %和(33.0 7± 3.73) % ;比 1mm宽缝合缘组试片的平均最大负荷 (7.0 5± 1.4 0 )N、平均极限抗张强度 (4 .6 2±1.0 8)mPa及断裂延伸率 (2 4 .11± 4 .4 0 ) %均明显提高 (均P≤ 0 .0 1) ,3mm组、4mm组的平均最大负荷及 4mm组的平均极限抗张强度亦高于 2mm组 (P <0 .0 5 ) ,其余组之间的各项检测值无差别。结论 根据其生物力学特性无支架四叶牛心包二尖瓣交界缝合缘的宽度至少应大于 1mm ,一般以 2mm为宜。
Objective:To investigate the suitable width of commissure suture of the stentless quadrileaflet bovine pericardial mitral valve (CS valve) in terms of biomechanical properties.Methods:Bovine pericardium strips tanned with glutaraldehyde were measured with thickness and cut off in the middle, then sewn up according to different distances from the sewing point to the cutting rim. The absolute load ,failure tensile stress and failure strain of strips were measured through uniaxial load testing. According to different suture width , the strips were divided into four groups: 1?mm width group, 2?mm width group, 3?mm width group and 4?mm width group.Results:In 2?mm, 3?mm and 4?mm groups, the absolute loads were (10.56±1.95)N、(13.57±3.27)N and (15.71±3.86)N, failure tensile stresses were (6.88±1.29)MPa,(8.25±2.58)MPa and (8.94±2.05) MPa and failure strains were(29.95±4.11)%,(33.31±5.02)% and (33.07±3.73)%.They were all significantly higher than those in 1?mm group which were (7.05±1.40)N,(4.62±1.08) MPa and (24.11±4.40)% respectively (P≤0.01).The absolute loads in 3?mm group and 4?mm group and the failure tensile stress in 4?mm group were also higher than in 2?mm group(P≤0.05).Other data showed no significant difference in all the groups.Conclusions:The width of commissure suture of the stentless quadrileaflet bovine pericardial mitral valve(CS valve) should be at least larger than 1?mm according to its biomechanical properties. Generally its suitable width is 2?mm.
