摘要
目的 :探讨树糖尿病模型肝脏功能及形态学变化与肿瘤坏死因子 (Tum or necrosis factor- alpha,TNF- α)的关系。方法 :用链脲佐菌素 (STZ)诱发树糖尿病模型 ,并于注药前及成模后 2、4、6周测血清 TBIL、AL T、TNF- α;6周肝活检行常规HE染色、PAS - DPAS染色、苏丹 染色、Masson染色及电镜检查 ,以及免疫组化检测肝组织 TNF-α。结果 :正常组及未成模组实验前后 FBS、TBIL、AL T、TNF-α浓度变化差异无统计学意义 (P >0 .0 5 ) ,肝组织 TNF-α免疫组化各有 1例阳性 ,统计学分析差异无统计学意义 (P >0 .0 5 ) ,光镜、电镜无异常发现。糖尿病组实验前后 AL T及 TBIL经统计学分析差异无统计学意义 (P>0 .0 5 ) ;血清 TNF- α浓度随实验时间延长逐渐升高 ,4、6周值与实验前比较差异有统计学意义 (P<0 .0 5 ) ;肝组织TNF- α免疫组化免疫组化发现 7例阳性。光镜下糖尿病组肝脏表现 :细胞质内糖原颗粒增多 ,细胞水肿 ,部分空泡样变 ,肝实质点状坏死 ,嗜酸性变 ,汇管区浸润。电镜下改变 :细胞质内糖原颗粒明显增多及脂肪变性 ,线粒体肿胀模糊电子密度增高 ,异染色质增多 ,粗面内质网脱粒 ,光面内质网增多。结论 :糖尿病树动物模型早期即可出现明显的肝脏病理改变 。
Objective:To study the relationship between the concentration of tumor necrosis factor-α(TNF-α) in serum and hepatic tissue and the histopathology damage in the liver of experimental diabetes tupaia(tree shrew).Methods:Tupaia were divided randomly into normal control group and experimental group. The experimental group was given STZ at the dosage of 120 mg/kg i.v.The normal control group was given acided normal saline. After 3 days, the tupaia were divided into diabetes mellitus (DM) group with FBG≥11.1 mmol/L, and non-diabetes group with FBG<11.1 mmol/l. The changes of FBG, ALT,TBIL and TNF-α before and after 2, 4 and 6 week treatment were tested respectively. At the 6th week of the experiment, liver tissue from biopsy were examined immunohistochemiscally for TNF-α level, and were observed with light microscope and electron microscope for pathologic changes.Result:There was no significant change in FBG, ALT,TBIL and TNF-α between the normal control group and the non-diabetes group. In DM group, the level of serum TNF-α was increased significantly compared to that before treated. The positive rate of TNF-α of liver tissue was significantly higher in DM group than that in the normal or control group (P<0.05). The liver histological damages were observed in DM group, light microscope observation showed glycogen granules increase in cytoplasm; hepatocytes swelling and punctate necrosis, acidophilic degeneration, and portal area infiltration. Electron microscope observation showed glycogen granules increase in cytoplasm and unclear, great deal of fatty droplets in the cytoplasm, decrease in mitochondrial with obscure and increase in electron density, decrease in rough endoplasmic reticulum, loss of granules,increase of smooth endoplasmic reticulum,interstitial collagenofiber increase.Conclusion:Hepatocytes damage was found in early stage of diabetes(6 week). There was no correlationship between ALT,TBIL and histopathology change. TNF-α was significantly high in blood and liver in DM group. The TNF-αmay plays an important role in diabetic hepatology in diabetes tupaia.
