摘要
目的观察锥形设计支架在颈动脉狭窄中的临床效果。方法回顾性研究了西安交通大学第一附属医院血管外科108例应用锥形支架治疗颈动脉狭窄患者的临床资料,观察其围术期并发症及术后30 d和6个月临床效果。结果平均狭窄率(82.50%±12.38%),均预扩后植入支架,其中后扩张率11.32%,总体技术成功率100%。锥形支架术中发生低血压、低心率等血流动力学并发症56例(低心率26例,低血压49例);术后发生低血压、低心率等血流动力学并发症37例(低心率16例,低血压23例)。术后并发症:高灌注8例(7.41%),穿刺口血肿3例(2.78%),无围手术期卒中、心肌梗死(MI)及死亡发生。术后30 d小卒中发生率1例(0.93%),术后6月总卒中发生率4例(3.70%),同侧卒中发生率2例(1.85%),非同侧卒中发生率2例(1.85%),无MI及死亡发生。结论锥形支架治疗颈动脉狭窄效果确切,并发症低,具有临床推广价值。
Objective To observe the clinical effect of conical design stent in carotid stenosis.Methods The clinical data of 108 patients with vascular surgery in the First Affiliated Hospital of Xi’an Jiaotong University were retrospectively studied.The perioperative complications and clinical outcomes at 30 and 6 months after operation were observed.Results The average stenosis rate(82.50%±12.38%)was pre-expanded and implanted.The post-expansion rate was 11.32%,and the overall technical success rate was 100%.In the conical stent,56 cases of hemodynamic complications such as hypotension and low heart rate occurred(26 cases of low heart rate and 49 cases of hypotension);37 cases of hemodynamic complications such as hypotension and low heart rate occurred after operation(There were 16 cases of low heart rate and 23 cases of hypotension.Postoperative complications included high perfusion in 8 patients(7.41%)and puncture hematoma in 3 patients(2.78%).There were no perioperative stroke,myocardial infarction(MI)and death.The incidence of small stroke was 1 case(0.93%)at 30 days after operation,4 cases(3.70%)of total stroke occurred in June,2 cases(1.85%)of ipsilateral stroke,and 2 cases of non-ipsilateral stroke.(1.85%),no MI and death occurred.Conclusion The conical stent is effective in the treatment of carotid stenosis with low complications and clinical value.
