摘要
目的 应用具有高频率的二维超声显像观察集束电极射频毁损治疗兔VX2肝肿瘤早期疗效。方法 超声引导下将20只大白兔种植VX2肝肿瘤模型,随机抽取4只作为对照组,余下16只作为治疗组进行射频治疗,应用频率为10 MHz的具有高分辨率的高频二维超声分别于治疗前1天和治疗后当天、第3天、第7天及第14天观察肿瘤形态学变化,并于治疗后每次超声检测结束处死4只治疗组大白兔,取肿瘤标本进行病理检查。结果 兔VX2肿瘤射频治疗后14天时治疗组肿块明显小于相应时间的对照组(P<0.01),治疗后治疗区呈极不均匀斑点状强回声及低回声,部分肿块周边仍有与治疗前肿瘤回声相似的区域。病理证实极不均匀回声区域即为肿块组织凝固坏死区域,局部仍有术前肿瘤回声特点的区域即为残存肿瘤或治疗不完全的区域。结论 兔VX2肿瘤射频治疗后凝固坏死区具有一定的早期声像图特征表现,应用高频率的二维超声显像观察其疗效具有十分重要的意义。
Abstract Objective To evaluate by ultrasonography the early effect of cluster electrode radio frequency (RF) ablation in treatment of VX2 tumor in rabbit liver. Methods Models of VX2 liver tumor were developed in 20 rabbits, which were divided into two groups at random, 16 as treatment! group undergoing RF ablation,and 4 as control group. Morphological features were investigated using high frequency two-dimensional ultrasound imaging 1 day before treatment,the same day and 3,7,and 14 days after treatment.Four rabbits in the treatment group were killed after each time of ultrasound imaging,and tumors specimens were subjected to pathologic studies. Results Sizes of tumors significantly decreased 14 days after treatment,compared with the control group ( P < 0.01). Spots of extremely inhomogeneous hyperecho and hypoecho were seen. In the periphery of part of tumors, there still existed hyperecho areas resembling those before treatment. Pathological study evidenced that the spots of extremely inhomogeneous echoes were coagulation necrosis zones, whereas the areas of localized areas of echoes resembling those before treatment were residual or inadequately treated tumors. Conclusions Eary ultrasound imaging features are preterit in the coagulation areas of rabbit VX2 tumor after RF ablation therapy,and high frequency two-dimensional ultrasonography is of particular significance in e-vahcation of the therapeutic effect.
