摘要
目的 探讨原发性青光眼患者心理应激的应对方式及采取不同应对方式对患者心理问题的影响。方法 采用“应对方式量表”对在我科住院的 13 8例原发性青光眼患者进行应对方式的测量 ;并将采取积极因素和消极两种应对方式的原发性青光眼患者分为两组 ,分别用“症状自评量表 (SCL 90 )”进行测评 ,比较两组患者的心理状况。结果 ( 1) 13 8例原发性青光眼患者对生活事件采取消极应对方式的人数明显高于积极应对方式人数 ;采取消极应对方式患者的年龄长于积极应对的患者 ;经统计学处理两者存在显著差异。 ( 2 )SCL 90总分及因子分比较 :对生活事件采取消极应对方式原发性青光眼患者的总分高于积极应对方式组 ,两组存在非常显著差异 ,其中 10个因子中有 9个因子消极应对组与积极应对组相比相差非常显著 (特别是忧郁、焦虑和阳性项目数两组有极显著的差异P <0 0 0 1) ,而有精神病性两组无显著差异。结论 ( 1)原发性青光眼患者对生活事件多采取消极应对方式 ,且原发性青光眼病随年龄增长人采取消极应对方式人数越多 ,反之 ,年龄偏小的原发性青光眼患者多采取积极的方式对待突发的生活事件。 ( 2 )原发性青光眼患者对心理应激的应对方式多寻求自已安慰自己、逃避、克制自己的情感或通过其他活动来解脱。 (
Objective To study the effect of different stress response modes to anxiety and psycho physical symptoms in patients with primary glaucoma.Methods According to the Response Mode Scale for Incident,138 patients with primary glaucoma were divided into 2 groups: positive group and negative group,and then psycho physical symptoms were measured with Symptom Checklist 90 (SCL 90) to compare the differences of psychological situation.Results (1) The number of patients in the positive group was much greater than that in the negative group and the age of the patients in the negative group was much older than those in the positive group (P<0.01).(2) SCL 90 results: the general score of the positive group was higher than that of the negative group.Nine of the 10 factors showed significant difference,especially in depression,anxiety and positive factors (P<0.001), though the psychopathogenic factors were not significantly different.Conclusion(1) The patients with primary glaucoma mostly took a negative response mode to the every day life incidents, and the ages related to the adopted modes: older, more negative, otherwise, younger,more positive.(2)The main mode of the patients with primary glaucoma responsing to incident ill stress is self comforting, escaping,inhibiting sensibilities and other avoiding methods.(3) A positive mode can lessen the psychological problems and has a positive correlation with mental health.
