摘要 目的 探讨静力型交锁髓内钉动力化治疗下肢骨干骨折延迟愈合的效果。方法 对 2 7例骨折应用静力型交锁髓内钉固定后的股骨和胫骨骨干延迟愈合患者 ,行近侧或远侧锁钉取出术进行动力化治疗。其中股骨干延迟愈合 17例 ,胫骨干延迟愈合 10例。结果 随访 7~ 4 8个月 ,平均 19 5个月。术后 3~ 7个月 (平均 3 3个月 )获临床愈合。无感染、断钉、关节损伤和内固定取出后再骨折。结论 静力型改为动力型是治疗交锁髓内钉固定后股骨和胫骨骨干延迟愈合的有效方法。应用时注意选择适当的手术时机 。 Objective To explore the effect of dynamization of static interlocking intramedullary nailing in the treatment of delayed union of diaphyseal fracture of lower extremity. Methods 27 cases of delayed union of diaphyseal fractures of femur (17 cases) and tibia (10 cases) were treated with dynamization. The proximal or distal screws of the primary static interlocking nails were removed. Results The follow up was from 7 to 48 months,average 19 5 months. All cases achieved bony healing between 3 to 7 months(average 3 3 months) after operation. No infection, nail break, joint injury, or refracture occured after dynamization. Conclusion Dynamization of static locking is an effective method to treat delayed union of femoral and tibial diaphyseal fractures fixed by static interlocking intramedullary nailing. The optimal time and the proper selection of locking screws for dynamization should be considered carefully.
