新兵足底筋膜炎患者足底压力分布与步态特征 被引量:12

Study on Plantar Pressure Distribution and Gait Characteristics in Recruits with Plantar Fasciitis

摘要 目的:探讨我国新兵军事训练中足底筋膜炎患者足底压力分布与步态特征。方法:运用比利时Footscan足底压力测试系统对19名患足底筋膜炎新兵(损伤组)和19名健康新兵(健康组)左、右足进行足底压力测试,其中损伤组左足为患侧,对获取的数据进行分析。结果:损伤组左足峰力值/体重在第1、5跖骨、足跟内侧和足跟外侧区域小于健康组(P<0.01),仅足弓区域大于健康组(P<0.05);着地达峰力值时间大于健康组的区域分别是第1趾骨、第1跖骨和足跟外侧区域(P<0.01);负荷率在第1趾骨和足跟内侧区域小于健康组(P<0.01),在第2~5趾骨、第1跖骨、足弓和足跟外侧区域负荷率小于健康组(P<0.05);在第1趾骨和足弓外侧区域损伤组冲量/体重小于健康组(P<0.01),足弓区域显著大于健康组(P<0.01)。损伤组左右足比较显示:左足第1、2跖骨和足跟内侧区域峰力值/体重小于右足(P<0.01);左足足跟外侧区域着地达峰力值时间大于右足(P<0.01);在第2~5趾骨,第4、5跖骨区域负荷率左足小于右足(P<0.05),左足足弓区域负荷率大于右足(P<0.01);左足第1跖骨区域冲量/体重小于右足,足弓区大于右足(P<0.01)。结论:新兵患病足各区域峰力值、负荷率值明显偏低,达峰力值时间较长,足跟和足弓接触地面时间长,表现出较高的冲量,而其它区域冲量值较小,这与新兵患病足由于疼痛和不适导致发力不足、承受力变小、前脚掌着地方式发生改变有关。 Objective To study the characteristics of plantar pressure distribution and gait characteristics of Chinese recruits with the plantar fasciitis during their military training.Methods The plantar pressure tests were performed on the injured left and healthy right feet of 19 recruits with plantar fasciitis(the injury group)and 19 healthy recruits(the healthy group)using the Belgian Footscan plantar pressure test system.The data were collected and analyzed.Results The value of the peak force/the body weight of the first and fifth metatarsal bones,heel medial and heel lateral areas of the left foot of the injured group was lower than that in the healthy group(P<0.01),but that of the arch area was higher than the healthy group(P<0.05).The time of landing peak force of the first phalanx,the first metatarsal bone and the heel lateral areas was significantly longer than that of the healthy group(P<0.01).The load rate in the medial areas of the first phalanx and heel,as well as the lateral areas of the second to fifth phalanx,the first metatarsal bone,the arch of the foot and the heel was significantly lower than that of the healthy group(P<0.01).The impulse/body weight of the first phalanx and the lateral area of the arch of the foot of the injured group was lower than that of the healthy group(P<0.01),while that of the arch area of the foot was significantly higher than the latter(P<0.01).In the injured group,the average value of the peak force/the body weight of the first and second metatarsal bone and the heel medial areas of the left foot was significantly lower than that of the right foot(P<0.01),while the time of landing the peak force in the heel lateral area of the left foot was significantly longer than that of the right foot(P<0.01).The loading rate in the areas of the second to fifth phalanx and the fourth and fifth metatarsal of the left foot was lower than that of the right foot(P<0.05),while that in the area of the arch of the left foot was significantly higher than that of the right foot(P<0.01).The impulse/the body weight of the first metatarsal area of the left foot was significantly lower than that of the right foot,and the arch area of the foot was larger than that of the right foot(P<0.01).Conclusions Compared with healthy counterparts,the peak force and load rate of recruits with plantar fasciitis are obviously lower,and the time of reaching the peak force is longer.Moreover,their heel and arch of the foot contact with the ground for a long time,showing higher impulse compared to smaller impulse value of other areas.This is related to the pain and discomfort caused by the recruits′sick feet,resulting in insufficient power,smaller tolerance,and changes in the way the forefoot hitting the ground.
