摘要
目的了解2017年菏泽市百日咳流行病学特征,为百日咳诊断、监测与预防控制提供科学依据。方法依据百日咳网络报告系统、个案调查与实验室检测等相关资料进行描述性研究。结果 2017年菏泽市共报告百日咳诊断病例88例,发病率0.9 397/10万,无死亡病例。病例相对集中又高度散发,分布于全市9县区的8县区的49个乡镇(办事处)的83个行政村(社区)。百日咳个案调查率100.00%。急性期血清标本采集率62.50%,急性期血清百日咳外毒素抗原(PT) Ig M阳性率67.27%,急性期血清PT Ig G阳性率83.64%;急性期鼻咽拭子标本采集率3.14%,均未培养出百日咳杆菌,均进行了实时荧光聚合酶链反应(Real-time PCR)检测,未检测到百日咳杆菌基因序列;百日咳实验室确诊率52.27%。结论 2017年菏泽市百日咳诊断、监测与预防控制取得了一些成绩,但仍存在不少问题。建议继续加强百日咳监测与百白破疫苗接种,尤其是要改进实验室检测。
Objective To understand the epidemiological feature of of pertussis of 2017 in Heze,and to provide scientific basis for the diagnosis,monitoring,prevention and control of pertussis.Methods Descriptive research was carried out according to the information of pertussis network report system,case investigation and laboratory test.Results In 2017,a total of 88 cases of pertussis were reported in Heze,with a incidence of 0.9 397/100 000 and no deaths.The cases are relatively concentrated and highly sporadic,distributed in 83 administrative villages(communities)in 49 towns(offices)in 8 counties of 9 districts of the city.The pertussis case investigation rate was 100.00%.The collection rate of serum samples in the acute phase was 62.50%,the positive rate of serum PT Ig M in the acute phase was 67.27%,and the positive rate of serum PT IgG in the acute phase was83.64%.The collection rate of nasopharyngeal swabs in the acute phase was 3.14%,and no pertussis bacilli were cultured.Real-time fluorescence polymerase chain reaction(Real-time PCR)detection was performed in all samples,and no pertussis bacilli gene sequence was detected.The laboratory diagnosis rate of pertussis was 52.27%.Conclusion In 2017,some achievements have been made in the diagnosis,monitoring and prevention and control of pertussis in Heze,but there are still many problems.It is recommended to continue to strengthen pertussis surveillance and pertussis vaccination,especially to improve laboratory testing.
