摘要
目的:评估静息态功能磁共振成像(RS-fMRI)在高血压偏头痛肝阳上亢分型中的价值。方法:选取24例高血压偏头痛患者(肝阳上亢证型11例、非肝阳上亢型13例)与24名健康对照者,行RS-fMRI检查,对静息态功能磁共振图像应用DPARSF2.2进行后处理。并用REST软件进行局部一致性分析(ReHo),观察默认网络并比较三组间ReHo值差异。结果:(1)偏头痛组和对照组ReHo值在左侧海马、左侧额叶直回、左侧中脑、右侧上外额叶、右侧额中回等脑区不完全相等(P <0.05,Alphasim校正)。(2)相比肝阳上亢组,非肝阳上亢组Re Ho值在海马旁回、梭状回、顶下小叶、左侧脑干、右侧小脑中脑脑区明显增高;在左侧大脑半球的壳核、颞中回、顶下小叶、额叶直回、扣带回等脑区ReHo值则有显著减低(P <0.05,Alphasim校正)。结论:肝阳上亢证与非肝阳上亢证高血压偏头痛患者在边缘系统、基底节区、壳核等脑区存在显著差异有助于我们更好的了解偏头痛的中医辨证分型。
Objective To explore the value of resting state functional magnetic resonance imaging(RS-fMRI) in the classification of hyperactivity of liver Yang in hypertensive migraine. Methods 24 hypertensive migraine patients(11 with hyperactivity of liver Yang and 13 without hyperactivity of liver yang) and 24 healthy controls were examined by RS-fMRI. The resting functional magnetic resonance images were pre-processed and post-processed by DPARSF 2.2. The local consistency analysis(ReHo) was performed with REST software to observe the default network and compare the ReHo values among the three groups. Results(1) The ReHo values in left marginal lobe, left frontal lobe, left cerebellum, right superior frontal lobe and right dorsolateral superior frontal gyrus were not completely equal in migraine group and control group(P < 0.05, corrected by Alphasim).(2) Compared with the non-hyperactivity of the liver-yang, in the cerebral hemisphere hippocampus, fusiform gyrus, parahippocampal gyrus, inferior parietal lobule, left posterior cerebellum and left brain stem the ReHo values were higher in the non-hyperactivity of liver-yang group, while those in the left cerebral hemisphere were significantly higher in the middle temporal gyrus, inferior temporal gyrus, inferior frontal gyrus, insular lobe, putamen, cingulate gyrus and so on. The ReHo value in the central anterior gyrus was significantly decreased(P < 0.05, Alphasim corrected). Conclusion Between the patients with hyperactivity of liver-yang syndrome and other brain regions and those without hyperactivity of liver-yang syndrome There are significant differences in limbic system, basal ganglia, frontal lobe.
