摘要
目的了解我国医疗机构失效模式与效应分析(Failure Mode and Effect Analysis,FMEA)管理工具的研究情况,探讨FMEA的研究热点主题和发展态势,为医疗机构风险管理实践工作提供参考。方法在中国学术期刊(网络版),以"FMEA"及其中英文同义词检索2018年4月以前发表的文献,描述文献发表的一般情况,对高频关键词共现矩阵进行系统聚类分析,结合主题词战略坐标探讨热点主题及其发展情况。结果共检索到文献559篇,剔除不相关文献后剩余531篇纳入分析。通过整理文献发现,国内医疗机构FMEA浙江等省份。研究热点内容集中在8个主题:手术室管理、输血输液安全管理、患者术后管理、ICU与气管拔管插管、流程管理、医院感染管理、患者跌倒、PICC。战略坐标中,手术室管理及患者术后管理、输液输血管理位于第一象限,向心度和密度较高,研究较为核心和成熟。结论近几年FMEA研究数量持续增加;研究内容丰富,热点集中于护理领域,各研究主题发展阶段不一;FMEA研究发展呈现学科、地域的不平衡。建议医疗机构积极探索不同主题的研究和实践,不同地区医疗机构之间加强相关研究合作。
Objective To analyze the research situation of FMEA management tools,to reveal the research hotspots and development trends,and to provide suggestions for the practice of risk management in medical institutions. Methods Literature review was made in database CNKI. FMEA and its co-words(both Chinese and English) was used to retrieve literature published before April,2018,and the general characteristics of published paper was described. Co-word cluster analysis was then performed on the co-occurrence matrix of high frequency key words to discuss the research hotspots and development in line with the subject strategic diagram. Results 559 literature were retrieved,and 531 of them were retained with those excluded irrelevant literature. The bibliometrics analysis showed that papers in FMEA field were in rapid growth,and the publication journals were concentrated in the nursing disciplines. The publication areas were concentrated in Guangdong, Jiangsu and Zhejiang Provinces, etc. These literature focused on 8 research hotspots,including operation room,transfusion safety management,postoperative management,ICU and trachea cannula and extubation,process management,hospital infection management,patient falls and PICC. In the subject strategic diagram, operation room, transfusion safety management and postoperative management were placed in the first quadrant,with a high degree of centricity and density,and the research was more core and mature. Conclusion FMEA research has been increasing in recent years. These articles cover a wide range of topics,research hotspots is concentrated in the nursing field,and each research topic are in different development stage. FMEA research shows the imbalance among subjects and regions. Medical institutions should actively explore the research and practice of different topics,and strengthen relevant research cooperation among different regional medical institutions.
