摘要
目的通过对医院门急诊药房药品调配差错类型的分析,以减少甚至杜绝差错的发生。方法对医院门急诊药房《药品调配差错登记本》中2010年5月1日至2013年4月30日期间登记的调配差错情况进行汇总分析。结果 3年内共发生调配差错185例,其中药品品种差错89例(48.11%),药品数量差错15例(8.11%),药物剂量剂型差错25例(13.51%),注射单填写差错6例(3.24%),收费差错5例(2.70%),包装相似差错14例(7.57%)。结论加强针对性管理,规范处方调配程序,制订优质服务规范,提高药学人员业务素质及工作责任心,可有效防范处方调配差错的发生。
Objective To analyze the drug dispensing error types in the hospital outpatient and emergency pharmacy to reduce and even prevent the error occurrence.Methods The dispensing errors recorded in the Drug Dispensing Errors Registration of the hospital outpatient and emergency pharmacy from the 1st May,2010 to 30th April,2013 were performed the summary and analysis.Results In these 3 years,185 cases of dispensing errors occurred,in which,89 cases(48.11%) were the errors of drug varieties,15 cases(8.11%) were the drug quantity errors,25 cases(13.51%) were the errors of dosage and usage,6 cases were the errors(3.24%) of filling in the injection list,5 cases(2.70%) were the charge errors and 14 cases(7.57%) were the errors of packaging similarity.Conclusion Strengthening the pointed management,standardizing the prescription dispensing procedures,formulating the first-rate quality service standards and improving the professional quality and responsibility of pharmacy staffs can effectively prevent the occurrence of prescription dispensing errors.
