摘要
目的探讨高血压伴不同水平的血浆同型半胱氨酸(homocysteine,Hcy)的患者的临床特点。方法选取2016年至2018年就诊于华北理工大学附属医院的原发性高血压患者共188例,按照血浆Hcy水平分为Hcy升高组(Hcy≥10μmol/L) 138例和非Hcy升高组(Hcy<10μmol/L) 50例。收集患者体质量指数(BMI)、诊室血压与心率、性别、吸烟、饮酒等一般情况,血常规指标,血脂指标,肾功能指标,心脏超声的相关指标,并计算E峰与A峰流速比值(E/A)与左室质量指数(LVMI)、左心室质量(LVM)、左房内径指数(LADI)、相对室壁厚度(RWT),动态血压指标。比较两组患者的上述临床资料。结果与非Hcy升高组比较,Hcy升高组患者男性比例更高,饮酒和吸烟比例更高,诊室收缩压相对较低,高血压病程较短,高血压家族史较少,肌酐、尿酸、尿素氮水平偏高,左室舒张末内径相对较大,RWT相对较小,E/A值相对偏高。结论血浆Hcy升高的高血压患者男性多见,肾脏损害及心脏结构改变更为严重。
Objective To investigate the clinical features of patients with hypertension with different levels of plasma homocysteine(Hcy).Methods A total of 188 patients with essential hypertension who were admitted to the Affiliated Hospital of North China University of Technology from 2016 to2018 were divided into two groups according to the level of Hcy:138 cases in Hcy-elevated group(≥10μmol/L)and 50 cases in non-Hcy elevated group(Hcy<10μmol/L).We collected their body mass index(BMI),office blood pressure and heart rate,gender,smoking,drinking,blood routine indicators,blood lipid index,renal function indicators,relevant indicators of echocardiography,and we calculated E/A and left ventricular mass index(LVMI),left ventricular mass(LVM),left atrial diameter index(LADI),relative wall thickness(RWT),ambulatory blood pressure indicators.Various clinical data were compared between the two groups.Results Compared with the non-Hcy elevated group,hypertensive patients in Hcy elevated group showed a higher ratio of men,drinking and smoking,the systolic blood pressure was relatively low,the duration of hypertension was shorter,family history of hypertension was less,levels of creatinine,uric acid,urea nitrogen were higher,left ventricular end diastolic diameter was relatively large,RWT was relatively small,and the E/A value was relatively high.Conclusion Hypertensive patients with elevated plasma Hcy are more common in men,and kidney damage and cardiac structural changes are more severe.
