摘要
目的 :探讨原发性高血压 (EH)患者尿 β2 微球蛋白 (β2 MG)的主要影响因素及其意义。方法 :选择不伴有糖尿病、原发性肾脏损害的EH患者 91例 ,采用放射免疫法测定早晨第 1次尿中 β2 MG ,将其分为 β2 MG正常 (对照 )和异常两组 ,并测定所有患者血压、血脂、空腹血糖及其他一般情况 ,进行统计分析。运用多元线性回归识别影响 β2 MG的主要因素。结果 :尿 β2 MG异常组 ,血清总胆固醇、三酰甘油、低密度脂蛋白胆固醇、脉压(PP)高于尿 β2 MG正常组 (P <0 .0 5 ) ,高密度脂蛋白胆固醇低于尿 β2 MG正常组 (P <0 .0 5 )。回归分析结果上述血脂各项指标及PP均未进入回归方程 ,只有年龄、病程及收缩压 (SBP)进入方程。结论 :在不伴有糖尿病及原发性肾脏疾病的EH患者中 ,脂质代谢紊乱及PP在早期肾脏损害中起一定作用 ,但不是主要的。影响早期肾脏损害的主要因素是年龄、病程及SBP。
Objective:To investigate the primary factors influencing β 2-microglobuminuria(β 2-MG) and its significance in essential hypertension(EH). Methods: Ninety-one EH without diabetes and primary kidney diease were included in this study. Radioimmunoassay was employed to determine β 2-MG in first urine in morning. Based on above results, the patients were divided into two groups: β 2-MG normal group(control group) and abnormal group.The other important data( blood pressure?serum lipid?FBG?age?course of disease?sex etc) were registered .Multivariate Linear regression analysis was performed to identify the most important factors of inflencing β 2-MG. Result: There were increase of TC, TG, LDL-C, pulse pressure(PP)(all P< 0.05)and decrease of HDL(P< 0.05) in β 2-MG abnomal group. Only age, course of disease and systolic blood pressure (SBP) entered. Conclusion: Our data suggest that the disorder of lipid metablism and the change of PP in EH without diabetes and primary kidney diease may play a role to the eary damage of kidney in EH, but they are not important. The most important factors are age, course of disese and SBP.
