摘要
目的分析血清中不同浓度的抗苗勒氏管激素(AMH)与卵子质量及妊娠结局的相关性,期望找到预测卵子或妊娠结局的良好指标。方法选择2015年10月-2017年6月宁夏医科大学总医院生殖医学中心年龄<38岁行卵泡浆内单精子注射患者106例,患者均采用长方案,在患者月经的第3天抽血检测AMH、促卵泡生成激素(FSH)、促黄体生成素(LH)及雌激素(E2),根据月经周期第3天中AMH的水平高低分为AMH低值组、AMH中值组和AMH高值组;详细记录卵子形态相关指标,统计每组患者的受精率、优质胚胎率和妊娠率并进行分组比较。结果 AMH中值组中正常卵子的比例62. 3%显著高于AMH低值组的44. 6%和AMH高值组的46. 2%(P<0. 01); AMH中值组中多个指标异常的卵子比例与AMH低值组和AMH高值组差异无统计学意义(P>0. 05)。AMH中值组的受精率94. 0%显著高于AMH低值组的83. 4%和AMH高值组的84. 2%(P<0. 01);而3组间的优质胚胎率及妊娠率差异无统计学意义(P>0. 05)。结论 AMH是预测卵子质量及受精率的良好指标,对胚胎质量及妊娠结局的预测意义不大。
Objective To analyze the correlations between different concentrations of serum anti-Müllerian hormone(AMH)and oocyte quality,pregnancy outcome,find good indexes to predict oocyte and pregnancy outcome.Methods From October 2015 to June 2017,106 patients(less than 38 years old)treated by intracytoplasmic sperm injection(ICSI)were selected from Center of Reproductive Medicine,the General Hospital of Ningxia Medical University,all the patients received long protocol.On the third day of menstrual cycle,the levels of serum AMH,follicle-stimulating hormone(FSH),luteinizing hormone(LH),and estradiol(E2)were detected.According to the level of serum AMH on the third day of menstrual cycle,the patients were divided into low level AMH group,middle level AMH group,and high level AMH group.The related indexes of oocyte morphology were recorded detailedly.The fertilization rates,high-quality embryo rates,and pregnancy rates in the three groups were compared.Results The percentage of normal occytes in middle level AMH group was 62.3%,which was statistically significantly higher than those in low level AMH group(44.6%)and high level AMH group(46.2%)(P<0.01).There was no statistically significant difference in the percentage of occytes with many abnormal indexes between middle level AMH group and the other two groups(P>0.05).The fertilization rate in middle level AMH group was 94.0%,which was statistically significantly higher than those in low level AMH group(83.4%)and high level AMH group(84.2%)(P<0.01).There was no statistically significant difference in high-quality embryo rate and pregnancy rate among the three groups(P>0.05).Conclusion AMH is a good index to predict oocyte quality and fertilization rate,but it is not a valuable tool to evaluate embryo quality and pregnancy outcome.
