经阴道三维超声对剖宫产瘢痕与非妊娠期异常子宫出血关系的评估 被引量:6

Evaluation of the relationship between cesarean section scar and abnormal uterine bleeding during non-pregnancy by transvaginal three-dimensional ultrasound

摘要 目的应用经阴道三维超声评估剖宫产瘢痕与非妊娠期异常子宫出血的关系。方法对2015年1月-2018年6月来该院检查的有剖宫产史的203例妇女进行阴道超声检查,根据瘢痕情况分为完整瘢痕组(116例)和瘢痕憩室组(87例),比较两组妇女异常子宫出血的发生率;测量瘢痕憩室组憩室的长径、深径、横径及憩室前方残存肌层厚度,并运用三维技术计算憩室容积,分析憩室相关超声参数与异常子宫出血的关系。结果完整瘢痕组中异常子宫出血14例,发生率12. 07%,瘢痕憩室组中异常子宫出血61例,发生率70. 11%,两组比较差异有统计学意义(P<0. 05);憩室的长径、深径、横径和容积在异常子宫出血与月经正常妇女中比较,差异有统计学意义(P<0. 05),而残存肌层厚度在二者间比较差异无统计学意义(P>0. 05)。结论剖宫产瘢痕憩室的存在与非妊娠期异常子宫出血高度相关,经阴道三维超声不仅能在二维图像上测量憩室大小,还能通过三维处理计算憩室容积,可作为二维超声诊断的补充,为临床医生的诊治提供更多的影像信息。 Objective Evaluate the relationship between cesarean section scar and abnormal uterine bleeding during non-pregnancy by transvaginal three-dimensional ultrasound.Methods From January 2015 to June 2018,Vaginal ultrasonography was performed on 203 women with a history of cesarean section.Scars are divided into two groups,complete scar group and keloid scar group with diverticulum.Compare the incidence of abnormal uterine bleeding between the two groups.Measure the long diameter and deep diameter of the diverticulum.Calculate the transverse diameter and the thickness of the residual muscle layer in front of the diverticulum,and calculate the volume of the diverticulum by three-dimensional technique.Analyze the relationship between the ultrasound parameters of the diverticulum and the abnormal uterine bleeding.Results There were 14 cases of abnormal uterine bleeding in 116 cases of complete scar group,the rate was12.07%,and 61 cases of abnormal uterine bleeding in 87 cases of keloid group,the rate was 70.11%.The difference between the two groups was statistically significant(P<0.05);the length of the diverticulum The diameter,depth,transverse diameter and volume were significantly different between normal uterine bleeding and normal menstruation(P<0.05),while the residual muscle thickness was not statistically significant(P>0.05).Conclusion The presence of cesarean section scar with diverticulum is highly correlated with abnormal uterine bleeding during non-pregnancy.Transvaginal three-dimensional ultrasound can not only measure the size of the diverticulum on two-dimensional images,but also calculate the volume of the diverticulum by three-dimensional processing,which can be used as a supplement for two-dimensional ultrasound diagnosis,and provide more image information for the clinician’s diagnosis and treatment.
