摘要
目的调查沈阳市某三甲医院2016年-2018年恶性肿瘤住院育龄妇女疾病构成,分析育龄妇女恶性肿瘤防治的薄弱环节。方法通过该医院病案管理系统提取2016年-2018年15岁~49岁女性住院患者病案首页资料,按照国际疾病分类(ICD-10)对主要出院诊断进行分类,采用SPSS20.0进行统计分析。结果该院2016年-2018年恶性肿瘤住院育龄妇女患者共8259例,恶性肿瘤住院育龄妇女例数占比呈现下降趋势(Trend-χ2=123.631,P<0.001)。恶性肿瘤疾病构成顺位前三位依次为子宫颈恶性肿瘤(23.71%)、乳腺恶性肿瘤(21.76%)和甲状腺恶性肿瘤(12.20%)。40岁及以上年龄段恶性肿瘤住院育龄妇女构成比最高,占比62.32%。甲状腺恶性肿瘤和白血病随年龄增加构成比呈现下降趋势,子宫颈恶性肿瘤和乳腺恶性肿瘤随年龄增加构成比呈现上升趋势。结论针对育龄妇女主要恶性肿瘤变化趋势,优化育龄妇女医疗资源配置,提高育龄妇女主要恶性肿瘤的诊治水平,做好三早预防,进一步降低恶性肿瘤住院育龄妇女病死率。
Objective To investigate the disease composition of women of childbearing age hospitalized with malignant tumors in a third-class A hospital in Shenyang from 2016 to 2018,and to analyze the weak links in the prevention and treatment of malignant tumors among women of childbearing age.Methods Home page data of female inpatients aged 15 to 49 years from 2016 to 2018 were extracted through the hospital medical record management system.The main discharge diagnoses were classified according to the International Classification of Diseases(ICD-10).SPSS20.0 was used for statistical analysis.Results From 2016 to 2018,8259 women of childbearing age were hospitalized with malignant tumors.The proportion of women of childbearing age hospitalized with malignant tumors showed a downward trend(Trend-χ2=123.631,P<0.001).The top three malignant tumors were cervical malignant tumors(23.71%),breast malignant tumors(21.76%)and thyroid malignant tumors(12.20%).The proportion of inpatients with malignant tumors aged 40 and over was the highest,accounting for 62.32%.The proportion of thyroid malignant tumors and leukemia decreased with age,while that of cervical malignant tumors and breast malignant tumors increased with age.Conclusion In view of the changing trend of main malignant tumors in women of childbearing age,we should optimize the allocation of medical resources for women of childbearing age,improve the level of diagnosis and treatment of major malignant tumors in women of childbearing age,do a good job in early prevention,and further reduce the mortality rate of inpatients with malignant tumors.
