临床分离泛耐药肺炎克雷伯菌对抗菌药物及消毒剂抗性的相关研究 被引量:6

Study on the antibiotic resistance and disinfectant resistance of extensive drug resistant Klebsiella pneumoniae isolated from clinical speciments

摘要 目的研究医院临床分离的肺炎克雷伯菌抗菌药物耐药性与消毒剂抗性水平的关系。方法采用药敏试验筛选泛耐药和敏感肺炎克雷伯菌,经MLST分子分型,测定最低抑菌浓度、最小杀菌浓度,分析其对消毒剂抗性水平的影响。结果经药敏试验筛选30株对常用抗菌药物几乎全部耐药的泛耐药菌和5株敏感菌,MLST分型显示泛耐药菌分为6个型,76. 67%为优势型ST11,5株敏感菌ST型各异。泛耐药菌中21株对碘伏MIC值为400 mg/L,26株对含氯消毒剂MIC值为300 mg/L,与大肠杆菌(8099)相同,其余均略低于标准菌株,5株敏感株对碘伏和含氯消毒剂MIC值均与标准菌株相同。结论与抗菌药物敏感菌株相比,临床分离泛耐药肺炎克雷伯菌对含氯消毒剂和碘伏消毒剂的抗性没有显著差异。 Objective To study the relationship between the antibiotic resistance and disinfectant resistance of clinical isolated Klebsiella pneumoniae.Methods The susceptibility test was used to screen the extensively-drug resistant and susceptible Klebsiella pneumoniae,and the evolutionary relationship was analyzed by MLST subtyping.The minimum inhibitory concentration and minimum bactericidal concentration was determined to analyze the level of disinfectant resistance.Results Thirty extensively-drug resistant strains revealed resistant to almost variou antibiotics and five susceptible strains were included for comparison.The extensively-drug resistant strains were divided into 6 types by MLST subtyping,and76.67%of the strains belonged to the dominant subtyping ST11.5 Sensitive strains revealed different ST types.The MIC value of 21 of the extensively-drug resistant strains was 400 mg/L for iodophor,and 26 had a MIC value of 300 mg/L for chlorine-containing disinfectant,which were equal to E.coli(8099).The MICs of the other resistant strains were less than E.coli(8099),and 5 sensitive strains showed the same MIC as E.coli(8099).Conclusion In comparison with antimicrobial-sensitive strains,the extensively-drug resistant Klebsiella pneumoniae revealed no tendency for the increasing resistance to chlorine-containing disinfectants and iodophors.
