摘要
目的验证长时程动态心电图Smartpatch对心律失常的临床诊断价值。方法选取2017年5月至2018年5月到普陀区中心医院就诊,临床表现为晕厥、黑矇、心悸的30例患者。24h动态心电图检测(Holter)后未明确诊断或未发现与症状相关的心律失常后使用Smartpatch记录14天,统计首次出现有症状和无症状的心律失常时间、并比较第1天、前7天、整个周期的心律失常阳性率之间差异及佩戴的舒适程度。结果 30例Smartpatch诊断出25例存在心律失常,而Holter检出6例(阳性率83.33%vs 20%,P<0.05)。首次出现症状性的心律失常时间平均3.46天,首次记录到的无症状心律失常时间为6.13天。前7天检出阳性率明显大于第一天[63.33%(19/30)vs 16.67%(5/30),P<0.001],但和整个监测周期的阳性率差异无统计学意义[63.33%(19/30)vs 83.33%(25/30),P=0.26],佩戴Smartpatch体感更舒适。结论 Smartpatch在心律失常患者中的应用是安全有效的,诊断率明显高于Holter检查。第1周的监测时间窗口具有重要的临床意义,但延长监测时间仍可以有重要的阳性发现。
Objective To verify the clinical diagnostic application of Smartpatch in arrhythmia.Methods A total of 30 patients with syncope,giddiness,and palpitations were selected from May 2017 to May 2018 in the Central Hospital of Putuo District.After 24 hHolter without confirming the diagnosis or finding any symptom-related arrhythmias,we used Smartpatch for 14 days to count the first symptomatic and asymptomatic time of arrhythmia and compare the difference positive rate of arrhythmia between the first day and the first seven days andthe whole monitoring period.Results Twenty-five cases were detected arrhythmia with Smartpatch and 6 cases with Holter(positive rate 83.33%vs 20%,P<0.05).The first occurrence of symptomatic arrhythmia was an average of 3.46 days,and the first recorded asymptomatic arrhythmia was 6.13 days.The positive rate detected in the first seven days was significantly bigger than that in the first day{63.33%(19/30)vs 16.67%(5/30),P<0.001],but there was no significant difference of positive rate between the first seven days and the whole monitoring period(63.33%(19/30)vs 83.33%(25/30),P=0.26)Conclusions The use of Smartpatch in patients with arrhythmia is safe and effective,and the diagnostic rate for arrhythmia is significantly higher than Holter’s.The monitoring time window of the first week has important clinical significance,but it can still have important positive findings when the monitoring time is extended.
