摘要
目的探讨手术室心理护理对宫颈癌患者术前应激反应及临床疗效的影响。方法选取2016年7月至2018年7月间汉中职业技术学院附属医院收治的111例宫颈癌手术患者进行回顾性分析,采用常规围手术期护理和个性化手术室心理护理的56例患者纳入研究组,仅采用常规围手术期护理的55例患者纳入对照组。比较两组患者的应激反应情况和临床疗效。结果研究组患者血压异常率和心率异常率均低于对照组,差异均有统计学意义(均P <0. 05)。研究组患者术前1d和术前30min的汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评分均低于对照组患者,差异均有统计学意义(均P <0. 05)。在疗效方面,与对照组比较,研究组患者疼痛持续时间短,下床活动时间早,肠道排气时间早,腹腔引流管拔除时间早,住院时间短,生活质量评分高,差异均有统计学意义(均P <0. 05)。结论手术室心理护理能有效改善宫颈癌患者术前生理应激和心理应激,提升患者的治疗依从性,保证术后治疗效果,为改善预后提供了保障。
Objective To investigate the influence of psychological nursing in operation room on the preoperative stress response and clinical efficacy in cervical cancer patients. Methods A total of 111 patients undergoing surgery for cervical cancer at Hanzhong Vocational and Technical College Affiliated Hospital from July 2016 to July 2018 were screened and restrospectively analyzed. Patients were divided into a study group( 56 patients) and a control group( 55 patients). The study group was given personalized psychological nursing in the operating room besides routine perioperative care and the control group was only given routine nursing. The stress response and clinical efficacy were compared between the two groups.Results The abnormal blood pressure and heart rate were significantly lower in the study group than in the control group( all P < 0. 05). Hamilton depression rating scale( HAMD) and Hamilton anxiety rating scale( HAMA) scores at 1 d preoperatively and 30 min preoperatively were significantly lower in the study group than in the control group( all P < 0. 05). The study group had a short duration of pain,an early out of bed activity,and intestinal discharge time compared with the control group. The exubation time was earlier,the length of hospital stay was shorter,and the quality of life was superior in the study group than in the control group( all P < 0. 05). Conclusion The implementation of psychological nursing in operating room can effectively improve the physiological stress and psychological stress in patients with cervical cancer before surgery,and improve the patient’s compliance with the treatment,guarantee the postoperative therapeutic efficacy,and provide guarantee for improving the prognosis.
