放疗加高温合并治疗恶性肿瘤(附90例分析) 被引量:3

Effect of Radiation Combined with Hyperthermia in the Treatment of Superficial Cancer in 90 Patients

摘要 自1980~1983年用放射加高温治疗(简称放热疗)晚期癌瘤90例,其中30例具2~3个病灶,90的共计124个病灶。具 2个病灶以上者,选最大的一个行放热疗,计93个病灶;较小的病灶则行单纯放疗(简称单放),计31个病灶。全组除2例属放射敏感的恶性淋巴瘤及尤文氏瘤外,其余均系中等或不敏感的鳞状细胞癌、腺癌和放射抗性的肉瘤。观察日期从治疗日开始,为4~36个月,其中19例3年,32例2年,28例1年,11例不足1年。放疗使用直线加速器8MV的X线、^(60)Co或深部X线治疗机。局部加热采用国产2450及915MH微波机。测温采用单针多点测温仪,加温中连续监测並记录组织内温度。放射剂量多数病人为4,000~5,000拉德(中位4,300拉德),每周1,000拉德。一般在放疗5~15分钟后加热(41~44C°),每次40分钟,加热10~12次做为一个疗程。放热疗的病灶平均面积比单放病灶大2倍以上,肿瘤全消率为53.8%;单放为29%。在同一患者身上更为明显,放热疗的CR为 67.7%,而单放为 29%。全组无严重并发症。 In 1980-1983 radiotherapy combined with hyperthermia (R+H) was used in 90 patients with superficial cancer. In each of 30 patients there were 2 to 3 separate lesions. In these cases R+H was applied to the biggest lesion while the smaller ones were gi- ven radiotherapy only. Hence 93 of 124 lesions received R+H and 31 lesions radiation alone. With the exception of 1 case each of malignant lymphoma and Ewing′s sar- coma, all of the lesions were of moderate sensitivity or resistant to radiotherapy (squamous cell carcinoma, adenocarcinoma and sarcoma). Duration of the follow-ups is 4-36 months, 19 cases for 3 years, 32 for 2 years, 28 for 1 year and 11 for less than 1 year. For radiotherapy 8 MV linear accelerator x-ray, ^(60)Co, and orthovoltage x-ray and for hyperthermia home made microwave machine of 2450 or 915 MHz were em- ployed. Temperature in the tumor tissue during heat application was determined and recorded with a device made at this Institute. Radiation dosage was 4,000-5,000 rad (median 4300 rad)and 1,000 rad/week, followed by 40 minutes of hyperthermia (41- 44℃) twice a week. 10-12 such sessions constitute a course of R+H. The lesions treated with R+H was more than double the size of those given radio- therapy only. In R+H group, CR occurred in 53.8% significantly better than that of radiation alone, 29%.
