细菌生物被膜对大肠埃希菌多药耐药的影响 被引量:2

The influence of BBF on multidrug resistance in Escherichia coli

摘要 目的探讨细菌生物被膜(Bacterial biofilm,BBF)对大肠埃希菌多药耐药的影响。方法收集2014年1月-2014年5月临床分离的大肠埃希菌108株,经23种抗菌药物药敏试验分离出多药耐药菌株63株,其中耐三类药物组10株、耐四类药物组26株、五类及以上耐药组27株。对各组菌株分别采用扫描电镜(SEM)、银染法、激光共聚焦显微镜(CLSM)方法进行细菌生物被膜检测。SEM、银染法检测BBF面积,CLSM检测BBF厚度。对SEM和银染法结果进行组织化学评分(H-score)。结果 SEM法与银染法耐三类药组、耐四类药组和耐五类及以上组BBF积分为(3.75±2.65)、(7.85±3.14)和(8.37±2.72)分与(4.13±3.60)、(7.61±2.94)和(8.13±3.07)分,三组两种方法的数据差异有统计学意义(P<0.05),同种方法三组间差异有统计学意义(P<0.05);而三组CLSM法BBF厚度分别为(35.67±6.01)、(46.73±9.55)、(52.38±13.81)μm,三组差异有统计学意义(P<0.05)。结论 BBF形成面积和厚度影响了细菌的多药耐药;随耐药种类的增多,耐药机制更为复杂,BBF影响力相对降低。 OBJECTIVE To explore the influence of BBF on multidrug resistance in Escherichia coli.METHODS We collected 108 strains of E coli clinically isolated from Jan.2014 to May 2014,from which 63 multidrug-resistant strains were isolated by 23 kinds of drug sensitivity tests,including 10 three-drug resistant strains,26 four-drug resistant strains,27 five or more-drug resistant strains.The BBF area of every strain was determined by scanning electron microscopy(SEM)and silver staining,and the thickness of BBF was determined by CLSM.H-score was applied to the assessment of SEM and silver staining results.RESULTS For SEM and silver staining,the BBF Hscore of the three-drug resistant strains group,four-drug resistant strains group,five or more-drug resistant group were(3.75±2.65),(7.85±3.14),(8.37±2.72)and(4.13±3.60),(7.61±2.94),(8.13±3.07)respectively.There were significant differences of BBF H-scores of the three groups determined by the two methods(P<0.05),and there were significant differences among the three groups determined by the same method(P<0.05).For CLSM,the mean BBF thickness of the three-drug resistant strains,four-drug resistant strains,and five or more-drug resistant group was(35.67±6.01)μm,(46.73±9.55)μm,(52.38±13.81)μm respectively,with significant difference(P<0.05).CONCLUSIONThe area and thickness of BBF affected the bacterial multidrug resistance,and with the increase of drug resistance,the mechanism of bacterial resistance was more complicated,and the influence of BBF was relatively lower.
