摘要
目的探究病毒性脑炎患儿肺部感染的病原学特点及影响因素。方法选择2014年1月-2017年12月西南医科大学附属医院神经内科及儿科重症监护室收治的病毒性脑炎患儿89例为研究对象,观察患儿肺部感染发生情况,单因素及多因素Logistics回归分析发生肺部感染的影响因素,采集患儿呼吸道样本,进行病原菌培养及鉴定,采用K-B法进行药敏试验。结果 89例患儿共有23例出现了肺部感染,肺部感染率为25.84%;23例肺部感染患儿共培养病原菌41株,其中革兰阴性菌24株占58.54%;患者脑电图(EEG)异常程度、发热时间>5 d、存在意识障碍、存在呼吸衰竭、住院时间≥1个月是导致病毒性脑炎患儿肺部感染的影响因素(P<0.05)。结论针对肺部感染的影响因素给予合理的干预措施,降低病毒性脑膜炎患儿预后不良的发生率。
OBJECTIVE To explore the etiological characteristics and influencing factors for pulmonary infection in the children with viral encephalitis.METHODS A total of 89 children with viral encephalitis who were treated in neurology department of Southwest Medical University Affiliated Hospital and pediatric intensive care unit from Jan 2014 to Dec 2017 were recruited as the study objects.The incidence of pulmonary infection was observed,the univariate analysis and multivariate logistic regression analysis were performed for the pulmonary infection,the respiratory tract specimens were collected from the children,the pathogens were isolated and identified,and the drug susceptibility testing was carried out by using K-B method.RESULTS Of the 89 children,23 had pulmonary infection,with the infection rate 25.84%.Totally 41 strains of pathogens were isolated from the 23 children with pulmonary infection,24(58.54%)of which were gram-negative bacteria.The degree of abnormal electroencephalogram(EEG),time of fever more than 5 days,disturbance of consciousness,respiratory failure and length of hospital stay no less than 1 month were the influencing factors for the pulmonary infection in the children with viral encephalitis(P<0.05).CONCLUSION It is necessary to take reasonable and targeted intervention measures according to the influencing factors for the pulmonary infection so as to reduce the incidence of adverse outcomes in the children with viral encephalitis.
