摘要 目的观察B7-H1蛋白在胆囊癌中的表达;探讨其在胆囊癌的发生发展中的作用及其与预后的关系。方法用免疫组织化学SP法检测B7-H1蛋白在60例胆囊癌组织中的表达;另取癌旁非肿瘤组织20例及胆囊腺瘤性息肉20例作为对照组。结果胆囊癌组中B7-H1蛋白的表达明显高于癌旁非肿瘤组织组(P=0.003)和胆囊腺瘤性息肉组(P=0.035);B7-H1蛋白在Nevin(Ⅳ+Ⅴ)期、TNM(Ⅲ+Ⅳ)期、有淋巴结转移组中的表达明显高于Nevin(Ⅰ+Ⅱ+Ⅲ)期(P=0.041)、TNM(Ⅰ+Ⅱ)期(P=0.049)、无淋巴结转移组(P=0.000);与性别、年龄及肿瘤分化程度无关(P>0.05)。B7-H1蛋白阳性表达组的三年生存期明显短于阴性表达者(P=0.029)。结论 B7-H1蛋白在胆囊癌中高表达导致预后不良,提示B7-H1蛋白在胆囊癌的发生发展和浸润转移中起作用。 Objective To investigate the expression of B7-H1 protein in gallbladder carcinoma tissues and to explore its role in the carcinogenesis and progression of gallbladder carcinoma. Methods The expression of B7-H1 protein in 60 cases of gallbladder carcinoma tissues were detected by immunohistochemistry(SP) method; 20 cases of corresponding non-tumor tissues and 20 cases of gallbladder adenoma polyp were served as control groups. Results The expression of B7-H1 protein in gallbladder carcinoma group was significantly higher than that in corresponding non-tumor tissues group(P=0.003) and gallbladder adenoma polyp group(P=0.035); the B7-H1 protein expression levels in Nevin(Ⅳ+Ⅴ) stage, TNM(Ⅲ+Ⅳ) stage and lymph node metastasis group were significantly higher than those in Nevin(Ⅰ+Ⅱ+Ⅲ) stage(P=0.041), TNM(Ⅰ+Ⅱ) stage(P=0.049) and lymph node metastasis-free group(P=0.000); but the expression of B7-H1 protein had no relationship with age, gender or tumor differentiation(P>0.05). The three-year survival time of the patients with positive expression of B7-H1 protein was significant lower than those with negative expression of B7-H1 protein(P=0.029). Conclusion The overexpression of B7-H1 protein results in poor prognosis, indicating that B7-H1 protein plays role in the development, invasion and metastasis of gallbladder carcinoma.
