摘要
目的:分析无创正压通气在小儿重症手足口病中的临床应用效果。方法:选择2018年6月-2019年2月本院儿科接收的重症手足口病患儿共200例,依据治疗方式将其分为有创组、无创组,每组100例,对比两组临床疗效、症状改善情况。结果:无创组治疗总有效率明显较有创组更高,差异有统计学意义(P<0.05);无创组血压、血糖、心率指标均较有创组更低,差异均有统计学意义(P<0.05);无创组呼吸频率、体温均低于有创组,住院时间、上机时间均短于有创组,差异均有统计学意义(P<0.05);治疗后,无创组PaO2、SpO2均高于有创组,而PaCO2低于对照组,差异均有统计学意义(P<0.05)。结论:采用无创正压通气能够有效治疗小儿重症手足口病,可降低气管插管率、呼吸机相关性肺炎的出现,降低医疗成本,缩短住院时间,改善患儿血压、心率、体温等水平,提高临床治疗效果,值得推广。
Objective:To analyze the clinical effect of noninvasive positive pressure ventilation in the treatment of children with severe hand,foot and mouth disease.Method:From June 2018 to February 2019,200 children with severe hand,foot and mouth disease were selected and divided into invasive group and noninvasive group according to the different methods of treatment,100 cases in each group.The clinical efficacy and symptoms improvement of two groups were compared.Result:The total effective rate of non-invasive group was significantly higher than that of invasive group(P<0.05).The blood pressure,blood sugar and heart rate of noninvasive group were lower than those of invasive group(P<0.05).The respiratory rate and body temperature of non-invasive group were lower than those of invasive group,and the hospitalization time and the time to go on the machine were shorter than those of invasive group(P<0.05).After treatment,PaO2 and SpO2 of non-invasive group were higher than those of invasive group,while PaCO2 of non-invasive group was lower than that of invasive group(P<0.05).Conclusion:Non-invasive positive pressure ventilation can effectively treat children with severe hand,foot and mouth disease,reduce the incidence of tracheal intubation rate,ventilator-associated pneumonia,reduce medical costs,reduce hospital stay,improve the level of blood pressure,heart rate,body temperature and other indicators,improve the clinical efficacy,it is worth of promoting.
