摘要
对7例植入人工耳蜗的聋人进行了双脉冲辨别和频率辨别试验,测定其残存听神经的功能状态。单脉冲电刺激加至蜗内电极时受试者均能听到一短促的声音;双脉冲时,如脉冲间隔大,可听到两个短声。双脉冲辨别阈(PPDT)是受试者能辨认出两个短声的最小脉冲间隔。结果表明,PPDT的范围是10~110ms,並与载波频率为250和500Hz时的频率辨别阈呈正相关关系(相关系数分别为0.893、0.878,P值均小于0.01)。作者认为PPDT的增大与听觉通路(主要是听神经)中的病理过程有关。10例听力正常的受试者,用耳机给声测定的PPDT为1.26±0.20ms(X±SD)。
Paired pulse discrimination test and frequency discrimination test were carried out in 7 totally deaf patients with cochlear prosthesis in order to evaluate functional condition of surviving auditory nerve. When a single pulse was delivered to the intracochlear electrodes, the patients could hear a brief sound. With a pair of pulses, so long as the interval between them was large enough, two brief sounds could be heard. Thus the paired pulse discrimination threshold(PPDT) is defined as the minimal interval between two pulses at which two separate sounds can be identified. The results showed that PPDTs ranged from 10-110ms and positively correlated with △Fs both at carrier frequencies of 250 and 500Hz(r = 0.893, 0.878, P<0.01). In 10 controls with normal hearing, PPDT was measured by presenting clicks through earphone with the results of 1.26±0. 20 ms(M±SD).The increased PPDT in the patients is considered to be related to the pathological process in the auditory pathway (mainly in the auditory nerve), from which the performance of cochlear prostheses in understanding speech might be deteriorated.
