摘要
目的 探讨星形细胞瘤动态增强MRI与血管内皮生长因子 (VEGF)表达的相关性。资料与方法 对 33例星形细胞瘤术前采用TurboFLASH序列进行动态增强扫描 ,通过对时间 信号强度曲线分析 ,获得对比剂首过期曲线的最大斜率 (SSmax)和Tm1两参数。术后取病检分别进行VEGF和微血管密度 (MVD)免疫组织化学染色 ,计算每一标本的VEGF表达程度及MVD。将VEGF表达与SSmax、Tm1分别进行相关分析。结果 VEGF不同表达程度的星形细胞瘤之间的SSmax或Tm1差异有统计学意义 (P <0 .0 5 )。星形细胞瘤的SSmax与VEGF表达程度成正相关 ,rs值为 0 .6 8;Tm1与VEGF表达程度成负相关 ,rs 值为 - 0 .6 97,P值均 <0 .0 0 1。VEGF表达程度与MVD存在相关性。结论 星形细胞瘤的VEGF表达程度与SSmax、Tm1之间存在显著性的相关 ,其病理生理学基础是VEGF促进星形细胞瘤血管生成。
Objective To discuss the correlation between dynamic contrast-enhanced MRI findings and vascular endothelial growth factor (VEGF) in astrocytomas.Materials and Methods Preoperative dynamic contrast-enhanced MR scanning with Turbo-FLASH sequence was performed in 33 patients with pathologically verified astrocytoma. The time-signal intensity curve was analyzed. The steepest slope (SS max) of the curve and the corresponding Tm1 in “first-pass” phase were obtained. After the surgery, the specimens were collected, and both the expression level of VEGF and the microvascular density (MVD) were measured with immunohistochemical technique. The correlation between the expression level of VEGF and SS max, Tm1 was studied. Results Significant difference in SS max, Tm1 was found among astrocytomas with different VEGF expression levels (P<0.05). SS max correlated well with VEGF expression levels (r s=0.68, P<0.001). Negative correlation existed between Tm1 and VEGF expression levels (r s=-0.697, P<0.001). Significant correlation existed between VEGF expression levels and MVD. Conclusion In astrocytomas, VEGF expression level bears a significant relationship to SS max and Tm1. Its pathologic base is that VEGF can precipitate the tumor angiogenesis. The analysis of the time-siAgnal curve of dynamic contrast-enhanced MRI is of help for predicting the VEGF expression in astrocytomas.
