LRP和GST-π表达与上皮性卵巢癌患者铂类耐药及预后的关系 被引量:8

Association of LRP and GST-π expressions with platinum-based chemo-resistance and prognosis in patients with epithelial ovarian cancer

摘要 目的 :探讨肺耐药蛋白(lung resistance protein,LRP)和谷光甘肽S转移酶π(glutathione S-transferase-π,GST-π)表达与上皮性卵巢癌患者化疗耐药及预后的关系。方法 :选取66例河北医科大学第四医院妇科2007年1月—2010年1月收治的上皮性卵巢癌患者的临床资料进行分析。所有患者均进行了全面分期手术或卵巢癌细胞减灭术,术后均完成了6~8个周期以铂类为基础的化疗。按照化疗疗效判定标准,分为敏感组及耐药组。采用免疫组织化学法检测66例上皮性卵巢癌组织中LRP及GST-π蛋白的表达,分析其与卵巢癌患者化疗耐药及生存的关系。结果 :66例上皮性卵巢癌组织中,LRP阳性者化疗有效率为61.2%(30/49),LRP阴性者化疗有效率为94.1%(16/17),2组间差异有统计学意义(P=0.011)。GST-π阳性者化疗有效率为61.7%(29/47),GST-π阴性者化疗有效率为94.7%(18/19),2组间差异有统计学意义(P=0.026)。在化疗耐药组中,LRP和GST-π阳性表达率分别为91.3%(21/23)和87.0%(20/23);在化疗敏感组中,LRP和GST-π阳性表达率分别为65.1%(28/43)和62.8%(27/43);2组间差异有统计学意义(P=0.020和P=0.039)。LRP阴性者中位生存期(overall survival,OS)长于阳性者(74个月vs 54个月,P <0.01),GST-π阴性者中位OS长于阳性者(66个月vs49个月,P <0.05)。结论 :LRP和GST-π的表达与上皮性卵巢癌患者的化疗耐药、疗效及OS有关。 Objective:To investigate the association of the expressions of lung resistance protein(LRP)and glutathione S-transferase-π(GST-π)with the chemoresistance and prognosis of patients with epithelial ovarian cancer.Methods:Sixty-six patients with epithelial ovarian cancer in the Fourth Hospital of Hebei Medical University were selected from January 2007 to January 2010,and their clinical data were analyzed.All patients had received complete staging surgery or cytoreductive surgery for ovarian cancer,and received postoperative platinum-based chemotherapy for 6-8 courses.According to the chemotherapy efficacy criteria,66 patients were divided into sensitive group and resistant group.The expressions of LRP and GST-πproteins in 66 cases of epithelial ovarian cancer tissues were detected by immunohistochemistry,and the relationship of LRP and GST-πexpressions with the chemo-resistance and prognosis of epithelial ovarian cancer patients was analyzed.Results:In 66 cases of epithelial ovarian cancer,the response rate to chemotherapy in LRP-positive patients was 61.2%(30/49),the response rate to chemotherapy in LRP-negative patients was 94.1%(16/17),the difference between the two groups was statistically significant(P=0.011).The response rate to chemotherapy in GST-π-positive patients was 61.7%(29/47),while the response rate to chemotherapy in GST-π-negative patients was 94.7%(18/19),the difference between the two groups was also statistically significant(P=0.026).In chemotherapy resistance group,LRP-positive expression rate was 91.3%(21/23),and GST-π-positive expression rate was 87.0%(20/23).In chemotherapy sensitive group,LRP-positive expression rate was 65.1%(28/43),and GST-π-positive expression rate was 62.8%(27/43).The differences between chemotherapy resistant and sensitive groups were statistically significant(PLRP=0.020,PGST-π=0.039).The median overall survival(OS)time of LRP-negative patients was longer than that of LRP-positive patients(74 months vs 54 months,P<0.01).The median OS of GST-π-negative patients was longer than that of GST-π-positive patients(66 months vs 49 months,P<0.05).Conclusion:The expressions of LRP and GST-πare associated with the chemotherapeutic resistance,efficacy and OS in patients with epithelial ovarian cancer.
