摘要
目的 研究椎间盘退变过程中 ,椎间盘退变的放射影像学与病理学改变。方法 选用 4 0只新西兰大白兔随机分为 2组 ,实验组切除兔腰椎间棘间、棘上韧带及棘突、关节突 ,造成力学失稳状态诱导形成椎间盘退变模型。术后一周、 3个月、 8个月时摄腰椎正、侧位X线片 ,观察腰椎影像学变化。第 3个月、 8个月时取腰椎间盘 ,进行组织检查 ,评定椎间盘退变的病理改变情况。结果 模型建立后 ,3个月、 8个月的X线片显现对照组无明显改变 ,实验组腰椎后突畸形 ,椎间隙狭窄 ,随着时间延长椎体软骨终板钙化更加明显。组织学观察发现 ,实验组随术后时间延长 ,髓核由椎间盘内脱出 ,并伴有椎间盘两侧软骨终板的纤维化即软骨终板发生退变。结论 椎体软骨终板的退变是椎间退变早期的主要表现方式。
Objective To study the alteration of radiology and pathology in the experimental degeneration of intervebral discs. Methods Forty New Zealand rabbits were randomly divied in 2 groups.In experimental group,lumbar spines supraspinal and interspinal ligament,spinous process and articular process were resected,in order to provide models of intervertebral discs degeneration induced mechanical instability state.Lumbar vertebraes X-ray photographs were taken at the first week,the third and eighth months after operation,in order to study lumbar vertebraes radiogical characterization.Lumbar intervertebral discs were removed at the third and eighth months after operation and examed in hixtology.So as to assess pathological change of the degeneration of intervertebral disics. Results There is no significant rakiological characterization at the third and eighth months after operation in controlled group lumbar vertebraes posterior processes deformation and intervertebral spaces stenosis,exist in the experimental group.Vertebral bodies cartilage endplates were calcified intensely with prolonged time.It was observed in histology that nucleus pulposus were prolapsed from intervertebral with intervertebral discs bilateral cartilage endplates fibrosis. Conclusion Vertebral bodies cartilage endplates degeneration is the main form of expression at the early stage of intervertebral disc degeneration.
